EMS training group: Participants receive electrical muscle stimulation using an electrical stimulator in the supine position, applied to the abdomen and back for 20 min, followed by the thighs for 20 min (40 min/day, 5 days/week, for 8 weeks, at home). The stimulation intensity is individually set at a tolerable level and gradually increased as tolerance improves. Participants maintain their usual lifestyle during the intervention period.
Control group: Participants receive no intervention and maintain their usual lifestyle, including physical activity and dietary habits, for 8 weeks.

Significant group x time interactions were observed in baPWV, FMD, and MoCA-J total and delayed recall scores. In the EMS group, baPWV decreased (p = 0.001), and FMD (p < 0.001), MoCA-J total score (p < 0.001), and delayed recall score (p < 0.001) increased significantly, whereas no significant changes were observed in the control group. After the intervention, the MoCA-J total score was significantly higher in the EMS group than in the control group (p = 0.020).

Thirty-one postmenopausal women (age, 59.2 +/- 7.2 years; EMS group, n = 16; control group, n = 15). All participants were non-smokers without regular exercise habits. No significant differences were observed between the groups in physical characteristics, arterial function, or cognitive function at baseline.
Assessed for eligibility, 45; excluded, 3; declined, 6. Randomized, 36 (EMS, 19; control, 17). Withdrew after allocation, 1 per group; excluded for missing data, EMS 2 and control 1. Analyzed, 31 (EMS, 16; control, 15).
