Objectives

Narrative objectives1

To verify the equivalence of sleep stage scoring, arousal events and respiratory events between the home PSG device Nightgraf and polysomnography (PSG) recorded simultaneously in patients with sleep-related breathing disorders, and to develop a deep neural network-based automatic scoring program for sleep stages, arousals and respiratory events specific to Nightgraf.

Assessment

Primary outcomes

Agreement of Nightgraf sleep stage scoring with PSG as reference (epoch-by-epoch accuracy and Cohen's kappa for 30-second epochs), agreement of the arousal index, and agreement of Nightgraf respiratory indices (3% ODI and 2-3% desaturation with arousal) with PSG-derived AHI and RDI. Total sleep time, sleep latency, sleep efficiency, sleep stage proportions, ArI, AHI and ODI are assessed by intraclass correlation coefficients and Bland-Altman analysis, and severity classification by AHI and ODI is assessed by ROC analysis. For the automatic scoring program, agreement with visual scoring by clinical laboratory technologists is assessed.

Key secondary outcomes

Correlation between subjective sleep assessment (interview and questionnaires including PSQI and ESS) and the severity of sleep disorders such as sleep apnea-hypopnea assessed by PSG

Other

Other related information

This is an observational study. Patients with suspected obstructive sleep apnea who are scheduled for diagnostic PSG at the Sleep Center of Inoue Hospital are invited, and those who provide written informed consent are enrolled. On the night of PSG, Nightgraf (forehead and chin electrodes, nasal cannula, abdominal sensor and SpO2 sensor) is attached together with the PSG sensors and both are recorded simultaneously overnight. Main observation items are patient characteristics, PSG and Nightgraf recording data, sleep questionnaires (PSQI, ESS and an S'UIMIN original questionnaire), periodic limb movement index, purpose of PSG, final clinical diagnosis and adverse events.
