This study aims to examine, using parallel mediation structural equation modelling (SEM), whether sleep disorders (ISI) act as a mediating variable in the effect of alarm stress on psychological distress (K-6) among nurses working in Japanese intensive care units.

This study aims to examine, using parallel mediation structural equation modelling (SEM), whether sleep disturbances (ISI) act as a mediating variable in the relationship between 'Alarm Coping', a measure of alarm fatigue, and psychological distress (K-6) among nurses working in Japanese ICUs. Furthermore, using an adjusted mediation model, we will explore whether the strength of the association between alarm stress and sleep/distress varies according to the level of alarm coping (statistical interaction) among nurses working in Japanese ICUs. In addition, we will examine the associations between each exposure and the K-6, ISI and intention to leave (TIS-J) using multiple regression and categorical analysis.

Individuals who have been informed about participation in the study and have given their free and informed consent
Those who have worked in the ICU for less than one month (as they have not had sufficient exposure to alarms)
Those on extended leave during the survey period (e.g. parental leave, sick leave, etc.)

Procedure for Statistical Analysis
(1) Descriptive Statistics
(2) Correlation Analysis
Correlation analysis shall be performed using Pearson's correlation coefficient for normally distributed variables and Spearman's rank correlation coefficient for non-normally distributed data. Bivariate correlations between each subscale of Alarm Stress and Alarm Coping and the K-6, ISI, and intention to leave shall be calculated using Spearman's rank correlation.
(3) Main Analysis: Mediation Analysis using SEM (Alarm Stress)
A parallel mediation model is constructed with Alarm Stress as the independent variable, ISI as the mediator, and K-6 as the dependent variable. Exposure is treated as a latent variable indexed by ordinal categorical items, taking measurement error into account. For each exposure, the total effect, direct effect, indirect effect and mediation rate are estimated, and the (Stress pathway and main) effects are evaluated separately.
(4) Secondary Analysis A: Mediation analysis using SEM (Alarm Coping)
We will construct a parallel mediation model with Alarm Coping as the independent variable, ISI as the mediator, and K-6 as the outcome. Both exposures will be treated as latent variables indexed by ordinal categorical items, taking measurement error into account. For each exposure, we will estimate the total effect, direct effect, indirect effect and mediation rate, and evaluate the (Coping pathway and control) groups separately. Taking into account the ordinal scale characteristics, robust estimation based on polychoric/polyserial correlations is employed, with WLSMV as the primary estimation method and ULSMV for sensitivity analysis.
