This pilot randomized controlled trial aims to evaluate the effects of stick-figure-based perceptual training on the accuracy of observational gait analysis (OGA) in physical therapy students. Specifically, the study examines three research questions: (1) whether stick-figure training improves accuracy of joint angle estimation compared with a control condition; (2) whether learning effects differ between vertical and non-vertical viewing conditions of stick-figure stimuli; and (3) whether training effects transfer to a clinically relevant task involving observation of actual patient gait videos. As a pilot trial, the study also seeks to inform the design of a subsequent confirmatory trial by estimating effect sizes and identifying methodological constraints.

Accuracy of joint angle estimation in observational gait analysis, measured as the absolute error (in degrees) between participants' estimated angles and the reference angles, assessed across nine indicators comprising three lower-limb joints (hip, knee, ankle) and three task conditions (stick-figure vertical condition, stick-figure non-vertical condition, patient gait video). Outcomes were measured immediately before and after the training session. Given the pilot nature of this trial, no single primary outcome was pre-specified; all nine indicators were treated as exploratory.

Participants in the training group performed an error-correction stick-figure training program in a single session. The program consisted of 150 trials total: 75 trials under the vertical condition and 75 trials under the non-vertical condition, with 25 trials per joint (hip, knee, ankle) in each condition. In the vertical condition, the basic axis of the stick-figure was always aligned vertically. In the non-vertical condition, the entire stick-figure was rotated by 10-20 degrees while preserving the target joint angle, displacing the basic axis from vertical. For each trial, a stick-figure stimulus was presented on a computer screen and participants estimated the joint angle. Immediately after each response, the correct answer was presented as feedback to facilitate error correction. Stick-figure stimuli were generated at predefined angles ranging from 0 to 40 degrees in 5 degree increments. Training stimuli were designed to be non-overlapping with pre- and post-test stimuli.

Participants in the control group received no training intervention. They were instructed to rest quietly for the same duration as the training session of the training group. No stick-figure stimuli, joint angle estimation tasks, or feedback were provided during this period. Pre- and post-test assessments were administered using the same procedures and stimuli as the training group, with an equivalent time interval between the two assessments.

Key inclusion criteria were as follows: (1) Final-year undergraduate students enrolled in a physical therapy program; (2) Visual acuity (corrected if necessary) sufficient for daily activities; (3) Understanding of the purpose, procedures, and potential risks of the study, and provision of written informed consent.

Key exclusion criteria were as follows: (1) Failure to meet any of the inclusion criteria; (2) Inability to participate during the study period.
