HbA1c significantly decreased from 7.95% [6.55-9.05] to 6.3% [5.8-7.2] at 12 months after initiation of tirzepatide (p < 0.001). Body weight and BMI were also significantly reduced. Improvements were observed in liver enzymes (AST, ALT) and lipid profile (LDL-C, HDL-C, triglycerides). A slight change in renal function (eGFR) was noted. Gastrointestinal symptoms were the most common adverse events.

A total of 82 patients with type 2 diabetes mellitus were included in this study. The median age was 55 years [49-66.5], with 55 males and 27 females. Baseline HbA1c was 7.95% [6.55-9.05], and BMI was 29.5 kg/m2 [26.9-34.4]. The median duration of diabetes was 8 years [4-14], and eGFR was 72 mL/min/1.73 m2[57-88]. Many patients were receiving concomitant medications, including SGLT2 inhibitors and biguanides.
A total of 101 patients who received tirzepatide at our institution were initially identified. After excluding 3 patients referred to other institutions, 3 who discontinued treatment on their own, and 13 who discontinued treatment, 82 patients with at least 12 months of follow-up were included in the final analysis.
Gastrointestinal symptoms were observed in 8 patients. Other events included weight loss (n = 1), treatment discontinuation due to financial reasons (n = 1), and refusal of injection (n = 1). Serious adverse events requiring hospitalization included cerebral infarction (n = 1), cholelithiasis (n = 1), osteomyelitis (n = 1), and nausea (n = 1).
The primary outcome was the change in HbA1c from baseline to 12 months after initiation of tirzepatide. Secondary outcomes included changes in body weight and BMI, liver enzymes (AST, ALT), renal function (eGFR), lipid profile (LDL-C, HDL-C, triglycerides), urinary albumin excretion (ACR), body composition (fat mass, muscle mass), incidence of adverse events, as well as changes in liver stiffness assessed by shear wave elastography (SWE) and hepatic steatosis assessed by attenuation imaging (ATI).
