This study is a scoping review and does not involve human participants; therefore, ethics committee approval was not required. However, to meet the input requirements of the UMIN registration system, the approval date of a related study was entered in the ethics approval field for reference.

1) Search strategy
A literature search will be conducted using the electronic databases Ichushi-Web (Japan), PubMed, CINAHL, and Web of Science. The search will primarily use the keywords "patient engagement," "hospitalized patients," and "fall prevention" (search date: March 3, 2026). Gray literature and hand searching will not be included.

2) Screening
All retrieved records will be imported into Rayyan. At least two reviewers will independently screen the titles and abstracts according to the eligibility criteria. Subsequently, at least two reviewers will independently assess the full texts and identify studies that meet the inclusion criteria. Any disagreements between reviewers will be resolved through discussion. If consensus cannot be reached, a third reviewer will be consulted, and the final decision will be made through discussion with the third reviewer.

3) Data extraction
Data will be extracted and organized from the included studies. The extracted items will include authors, year of publication, country, study aim, study design, study setting, sample size, patients' clinical characteristics, and perceptions and behaviors related to patient involvement in fall prevention during hospitalization.
