(1) Physicians qualified as family medicine specialists
(2) Physicians in hospitals without a palliative care specialist
Key exclusion criteria
(1) None
(2) Physicians affiliated with a base hospital for cancer treatment, a facility notified as a palliative care ward, a single department psychiatric hospital, or a hospital with physicians specializing in or certified by the Japanese Society for Palliative Medicine.

Results
Primary care physicians in clinics required less consultation than those in hospitals on the choice of medication (P = .019), opioids switching, prognosis estimates, decision support, and grief care . Those in clinics were less likely to have palliative care support from non-physician palliative care specialists and information support. In multivariable analysis, being a clinic-based physician was inversely associated with the functioning of a decision-making support counseling system.
