本邦では視覚障害者手帳取得者の原因疾患において緑内障,網膜色素変性症,糖尿病,加齢黄斑変性が多くを占めており,時に進行性の視機能低下(ロービジョン)を起こす.視覚障害者数は増加傾向であり,高齢化に伴い今後も増加すると考えられる.
　白内障は本邦で年間160万件以上行われており,費用体効果の高い手術であると報告されている.また,白内障手術により患者の認知機能,抑うつ,視覚関連の生活の質が改善したとの報告もある.それらの報告でも術後視力値と満足度が有意に相関していたと報告している.一方,術後も視力改善が限定的な患者に限定した報告は無く,手術適応に関するコンセンサスはない.

In Japan, the leading causes of visual disability among holders of the official vision impairment certificate are glaucoma, retinitis pigmentosa, diabetic retinopathy, and age-related macular degeneration, all of which may result in progressive visual deterioration (low vision). The number of individuals with visual impairment is increasing, and this trend is expected to continue with the aging of the population.

Cataract surgery is performed more than 1.6 million times annually in Japan and has been reported to be a highly cost-effective procedure. Previous studies have also demonstrated that cataract surgery improves patient`s cognitive function, depressive symptoms, and vision-related quality of life. These reports further showed a significant correlation between postoperative visual acuity and patient satisfaction. However, no studies have specifically focused on patients whose postoperative visual improvement remains limited, and no consensus has been established regarding surgical indications in such cases.
