目的1/Narrative objectives1

日本語
本研究では、食道運動障害患者に対する低侵襲手術であるPOEM(Peroral Endoscopic Myotomy)施行時に、約2mm大の食道体部の内輪筋を3か所(エントリー部、中間部、LES部)から内視鏡生検で採取し、得られた組織について、RNAシーケンスおよび免疫染色を行う。RNAシーケンス結果および免疫染色結果を、食道内圧測定検査の結果と比較解析する。本研究の目的は、食道内圧測定検査の個々の所見について、分子病理学的なメカニズムを明らかにすることである。

In this study, during POEM (Peroral Endoscopic Myotomy) for patients with esophageal motility disorders, endoscopic biopsies of the endoluminal muscles of the esophageal body, approximately 2 mm in size, are obtained from three locations (entry, middle, and LES). The obtained tissues are subjected to RNA sequencing and the results of RNA sequencing and immunostaining will be compared with the results of esophageal manometry. The purpose of this study is to elucidate the molecular pathological mechanisms of individual findings of esophageal manometry.

研究成果により食道内圧測定検査の個々の所見に対応する分子病理学的異常が明らかとなれば、食道運動障害患者における新たな診断法や、治療選択肢を提示できる可能性があると考えられる。

採取した部位の食道内圧測定検査の異常所見(正常収縮、過剰収縮、収縮不全、弛緩不全)と、RNAシーケンス検査結果、免疫染色結果を比較する。

Abnormal findings (normal contraction, excessive contraction, contraction failure, and relaxation failure) of the esophageal manometry at the site of collection will be compared with the results of the RNA sequencing test and immunostaining results.

選択基準/Key inclusion criteria

①岡山大学病院に通院または入院している食道内圧測定検査を実施済みまたは予定されている食道運動障害患者で、研究機関の長の許可日から2028年12月31日の間にPOEMを実施する患者
②自由意思による研究参加の同意を本人から文書で取得可能な患者
③同意取得時の年齢が18歳以上90歳未満の患者

1) Patients with esophageal motility disorder who have already undergone or are scheduled to undergo esophageal intra-esophageal manometry who are attending or hospitalized at Okayama University Hospital and who will undergo POEM between the date of approval by the head of the research institution and December 31, 2028
2) Patients who can obtain free and voluntary written consent for research participation from the patient
3) Patients between 18 and 90 years of age at the time consent is obtained
