目的1/Narrative objectives1

日本語
切除可能進行大腸癌の治療は現在手術および術後補助化学療法が標準治療となっているが、大腸癌手術後には20～30％程度の合併症が発生する。過去に我々は、JFMC7/JFMC15/JFMC33の症例群を用い、大腸癌術後の合併症の発生が大腸癌術後の予後に影響を与える重要なリスク因子であることが報告した。しかしながら、大腸癌術後の合併症がどのようなメカニズムで予後に影響を与えるかは明らかにはできなかった。一方で、他の消化器癌の知見から消化器癌術後の合併症などの有害事象の発生が術後の補助化学療法の施行や継続に影響を及ぼすことが報告されている。このため、今回、JFMC37/JFMC38/JFMC41の3つの試験を用いて大腸癌術後合併症の発生が術後補助化学療法の継続及び施行に影響するか否か、その結果予後に影響するか否かを検討し、大腸癌術後合併症の臨床的意義に関して検討を行う。

Surgery and postoperative adjuvant chemotherapy are currently the standard treatments for resectable advanced colorectal cancer, but postoperative complications occur in 20-30% of patients after colorectal cancer surgery.
In our previous study, we reported that the occurrence of postoperative complications after colorectal cancer surgery is an important risk factor affecting the prognosis after colorectal cancer surgery using the case series of JFMC7/JFMC15/JFMC33.
However, we could not clarify the mechanism by which postoperative complications after colorectal cancer surgery affect the prognosis.
On the other hand, findings from other gastrointestinal cancers have reported that the occurrence of adverse events such as postoperative complications in gastrointestinal cancers can affect the implementation and continuation of adjuvant chemotherapy.
Therefore, in this study, we will investigate whether the occurrence of postoperative complications in colorectal cancer affects the continuation and implementation of adjuvant chemotherapy and, consequently, prognosis, using three studies, JFMC37/JFMC38/JFMC41, and discuss the clinical significance of postoperative complications in colorectal cancer.
