日本語
HER2陽性の根治切除不能な進行・再発の唾液腺癌に対するハーセプチン注射用60，同150（以下，本剤）の使用実態下における副作用の発現割合を把握すること。
安全性検討事項：心障害，Infusion reaction，間質性肺炎・肺障害，血液毒性，肝不全・肝障害，腎障害，昏睡・脳血管障害・脳浮腫，感染症，腫瘍崩壊症候群

To grasp the incidence of adverse drug reactions when HERCEPTIN for intravenous infusion 60 and 150 (hereinafter, HERCEPTIN) is used for advanced or recurrent HER2-positive salivary gland cancer not amenable to curative resection, under conditions of actual clinical use.
Safety specifications: Cardiac disorders, infusion reactions, interstitial pneumonia / lung disorders, hematologic toxicity, hepatic insufficiency / hepatic disorders, renal disorders, coma / cerebrovascular accidents / cerebral edema, infectious diseases, tumor lysis syndrome

主な調査項目等
1）施設情報 施設名，科名，調査票記入年月日，調査票記入医師名
2）患者背景 患者イニシャル，投与開始時年齢，性別，妊娠の有無，識別番 号，使用理由（本適応の場合，HER2過剰発現・遺伝子増幅検査 結果），臨床診断名，身長，体重，Performance Status（ECOG PS）， 悪性度，初回診断時の病期分類，治療ライン，本剤投与時点の 転移巣の有無，有の場合：転移部位，既往歴，合併症
3）前治療歴 原疾患に対する手術の有無，有の場合：詳細 原疾患に対する放射線療法の有無，有の場合：照射方法 原疾患に対する化学療法の有無，有の場合：薬剤名
4）本剤投与状況 投与年月日，1回投与量，観察期間終了時の本剤の状況（継続中， 休薬，中止（中止の場合，中止理由））
5）併用化学療法（ドセタキセル製剤） 投与年月日，1 回投与量
6）有害事象 有無，有の場合：有害事象名，発現日，最悪時 Grade，重篤度， 処置（本剤，その他），転帰，転帰日，因果関係（本剤，他の要 因），有害事象のうち安全性検討事項に関連する臨床検査値，有 害事象に関連する併用被疑薬

Main survey items etc.
1) Information on facilities: Name of facility, name of department, case report form entry date, name of physician who fills out the case report form
2) Patient demographics: Patient initials, age at administration start, sex, pregnancy status, identification number, reason for use (if used for this indication, HER2 over expression or gene amplifications test results), name of clinical diagnosis, height, weight, performance status (ECOG PS), malignancy, stage classification at initial diagnosis, treatment line, presence / absence of metastatic lesions at the time of administration of HERCEPTIN, if present: metastatic site, previous medical history, complications
3) Prior treatment history: Details of surgery for the primary disease, if performed. Irradiation method, if radiation therapy for the primary disease was undertaken. Name of medications, if chemotherapy for the primary disease was undertaken.
4) Administration status of HERCEPTIN. Administration date, amount in a single dose, status of HERCEPTIN at the end of the observation period (ongoing, temporarily suspended, discontinued [if discontinued, reason for discontinuation])
5) Combination chemotherapy (docetaxel preparation): Administration date, amount in a single dose
6) If adverse events occur: Name of adverse event, date of onset, grade at the worst point, severity, treatment (HERCEPTIN, other), outcome, date of outcome, causal relationship (HERCEPTIN, other cause), laboratory test values related to the safety specifications among adverse events, co-suspect drugs related to the adverse event

登録対象症例：本調査契約締結施設において，登録期間中に本剤を使用予定である HER2 陽性の根治切除不能な進行・再発の唾液腺癌の患者。
調査票回収対象症例：登録対象症例のうち，本剤を使用したすべての患者。

Target cases for enrollment: Patients with advanced or recurrent HER2-positive salivary gland cancer not amenable to curative resection to whom HERCEPTIN is scheduled to be administered during the enrollment period in facilities where a contract for this surveillance has been concluded.
Target cases for case report form collection: All patients from among the target cases for enrollment who use HERCEPTIN.
