治療開始4週目のFDG PET/CT評価と治療開始16週目のRECIST評価（奏効率）の相関
治療開始4週目評価の定義
Responder:治療開始4週目において新規病変を認めず、
maxSUVaxが20%以上の上昇または腫瘍径の総和が20%以上低下した症例
Non-responder: 上記のResponderに該当しない症例

CTにて腎癌病変と判断された全病変のStandardized Update Value (SUV)を測定する。対象症例の全ての腎癌病変のうちで最も高いSUVをmaximum SUVmax (max SUVmax)と定義し、これを特定する。

To test the correlation between judging by FDG PET/CT at 4 weeks after treatment start and the evaluation of therapeutic effect based on RECIST using CT scan at 16 weeks.

The criteria of judging by FDG PET/CT at 4 weeks
Responder: the case who satisfies the following conditions
1) max SUVmax increases more or 20%
2)without new lesions
Non-responder: the case who does not meet "responder"

max SUVmax: the highest SUV in the individual patient

本研究は1アームの多施設前向き第Ⅱ相試験である。日常診療で通常使用されるニボルマブ投与予定の患者のみを対象とした試験であり、治療に関しては非介入であるが、治療開始後の効果判定のFDG PET/CTは日常診療を超えた行為であるため、介入行為と考える。

Design of this study is prospective, multi-centered, single arm, interventional phase II study. Only the patients who are planned to receive nivolumab treatment are enrolled in this study, and the treatment with nivolumab is performed within the label of the drug as daily medical practice. Therefore the study is non-interventional in terms of treatment with nivolumab. However, because the second FDG-PET/CT for evaluation of treatment efficacy is beyond the daily medical practice, this study is interventional.

選択基準/Key inclusion criteria

以下の全てをみたす腎細胞癌患者
(1)登録時年齢が20歳以上
(2)病理組織学的に腎細胞癌の成分が確認されている
(3)ニボルマブ投与予定である
(4)RECIST v1.1で規定されたCTによる評価可能病変が少なくとも1つ存在する
(5)本臨床試験に先立って、患者本人から文書による同意が得られている患者

Patients who meet all of the following are eligible.
(1) Over 20 years old
(2) Histologically confirmed renal cell carcinoma
(3) Received the treatment by nivolumab
(4) More than one target lesion defined by RECIST (v1.1)
(5) With written informed consent

除外基準/Key exclusion criteria

以下のいずれかに該当する患者
(1)コントロール不良な糖尿病（空腹時血糖>150mg/dL）を合併する患者
(2)臓器移植の治療歴（造血幹細胞移植を含む）のある患者
(3)悪性腫瘍の既往を有する患者（但し、適切に根治的治療を施行された上皮内子宮頸癌、基底細胞癌、表在性膀胱腫瘍［Ta、TisおよびT1］、および、本臨床試験の3年以上前に根治的治療が施行され、その後再発していないと判断される悪性腫瘍を除く）
(4)妊娠中、授乳中、または妊娠している可能性がある患者
(5)ニボルマブ以外の抗悪性腫瘍治療および抗腫瘍効果を目的とした治療薬を併用している患者
例）化学療法、ホルモン療法、免疫療法、抗体療法、放射線療法、温熱療法、手術療法等
(6)その他、医師の判断により対象として不適当と判断された患者

Patients who meet any of the following are excluded from the study.
(1) Poorly-controlled diabetes mellitus (fasting blood glucose >150mg/dL)
(2) History of organ transplantation (including bone marrow transplantation)
(3) History of malignancy except:
(i) Curatively treated intraepithelial cervical cancer, basal cell carcinoma, superficial bladder cancer (Ta, Tis and T1).
(ii) Patients who had been disease free more for than 3 years after curative therapy
(4) Pregnant and/or nursing woman, possibility of pregnancy
(5) Patient treated simultaneously by other antineoplastic therapeutic or therapy purposing antitumor effect except for nivolumab
examples: chemotherapy, hormonal therapy, immunotherapy, antibody therapy, radiotherapy, hyperthermia, surgery
(6) any other cases who are regarded as inadequate for study enrollment by investigators
