頭頸部扁平上皮癌に対して放射線化学併用療法の効果に応じてその後の治療法を振り分ける（chemoradioselection）アルゴリズムに基づく集学的治療の実態および有効性・安全性を、多施設共同研究による第II相試験により本アルゴリズムの有効性・安全性を検討する

選択基準/Key inclusion criteria

日本語
組織学的、細胞学的に確定診断が得られている頭頸部扁平上皮癌である
原発巣が喉頭癌、中咽頭癌、下咽頭癌のいずれかである
ECOG Performance Status（PS）：0-2
主要臓器機能が保持されており、以下の基準を満たす
白血球数>=3500
血小板数>=100000
クレアチニンクリアランス>=30mL/min
十分な説明を受け本人の文書による同意が得られている
避妊への同意（妊娠の可能性のある女性、生殖能力のある男性）が得られている

Histologically or Cytological proven squamous cell carcinoma of the head and neck
Primary lesion located at oropharynx, hypopharynx, or larynx
ECOG performance status 0 to2
No history of irradiation to head and neck
Adequate organ function including;
WBC>=3,500/mm3
platlet>=100,000
Creatinin clearance>=30 (Cockcroft-Gault formula)
Written informed consent
Women of child-bearing potential and men who are able to father a child agree with using adequate contraception

除外基準/Key exclusion criteria

一時治療としてCRT,BRTを拒否する症例
科学放射線治療では制御困難であり、機能回復も困難であると考えられる広範な高等軟骨破壊を認める喉頭癌/下院東岸T4a 症例、および手術不能である喉頭癌/下咽頭癌T4b症例
遠隔転移を認める症例
同時性重複癌症例（内視鏡下粘膜下層剥離術や内視鏡下粘膜切開術で治療可能な食道癌症例を除く）
重篤な心疾患、心不全
6か月以内の心筋梗塞と狭心症発作
担当医師が参加不適当と判断した症例

Patients who reject chemoradiotherapy or bioradiotherapy as initial therapeutic intervention
T4a cases with extensive distruction of laryngeal framework and in-operable T4b cases
Distant metastasis cases
Sever heart disease or heart failure
Myocardial infarction or angina attack within 6 month
Cases with Synchronous multiple primary cancer other than esophageal cancer which can be treated by Endoscopic Submucosal Dissection or endoscopic mucosal resection
Other significant disease that in the investigator's opinion would exclude the subject from the trial.
