目的/Objectives
目的1/Narrative objectives1	この登録研究の目的は、がん患者の網羅的遺伝子解析を通じて、がんの治療可能な標的の発見と臨床データを合わせたリアルワールドエビデンスの構築である。	The purpose of this registrative study is to identify actionable genomic alterations by genomic sequencing and build the registry that could be used for generating real-world evidence.

評価/Assessment
主要アウトカム評価項目/Primary outcomes	以下の項目がデータベースに登録される。 a. 患者背景 （年齢、性別、既往歴、家族歴、社会歴、生活歴） b. 診断 （診断名、組織型、原発部位、転移巣、ステージ） c. 治療 （治療方法、治療薬、治療効果、治療効果持続期間、副作用、QOL） d. 遺伝子解析結果 （解析方法、検査の質、遺伝子名、変異の種類、変異の詳細など） e. 予後 （全生存期間、無病生存期間、無増悪生存期間、無再発生存期間など）	The following items will be recorded in the registry. a. Demographic (age, sex, co-morbidities, family history, social history) b. Diagnosis (tumor type, histology, primary site, metastatic site, stage) c. Treatment (modality, regimen, response, duration of response, adverse event, QOL) d. Genomic alteration (method, QC, gene name, alteration type, details of alteration etc.) e. Outcome (overall survival, disease-free survival, progression-free survival, relapse-free survival etc.)

選択基準/Key inclusion criteria	- がんまたはがん関連疾患の診断がされ、かつ告知されている者 - 次の診断を受けている者 a) がんの標準治療に不耐または治療抵抗性となり、それ以上の標準治療がない、または、がんの標準治療を受けているが、その標準治療に不耐または治療抵抗性になると、その後は治療法が残されていない b) 原発不明がん c) 希少がん	a) any cancer or malignancy that becomes refractory/intolerant to standard thearpy b) cancer of unknown origin c) rare cancer
除外基準/Key exclusion criteria	- 本人より文書にて同意が得られない者	- those who cannot consent
