CTL peptideである126ペプチドと235ペプチド、およびHTL peptideである332ペプチド(各2mg)より構成される3種混合WT1ペプチドワクチン (WT1 Trio）/Montanide ISA51　アジュバントを2週間毎に7回皮内投与する。

A patient is vaccinated with Montanide ISA51-adjuvanted WT1 Trio cancer vaccine consisting of two WT1 CTL peptides (WT1-126 and WT1-235) and one WT1 HTL peptide (WT1-332) (2mg each) seven times at the interval of two weeks.

1 胸腺癌　・胸腺腫　（Stage IVAおよびIVB）
軟部肉腫　（再発、進行期）＊子宮肉腫、乳腺葉状肉腫を含む
Grade III IV 悪性神経膠腫　（再発・進行期)
唾液腺癌　（再発、進行期）
頭頸部粘表皮癌（再発進行期）
嗅神経芽細胞腫　（再発・進行期）
頭頸部悪性粘膜黒色腫　（再発・進行期）
眼原発悪性黒色腫（再発、進行期）　
聴器癌　（再発、進行期）　
精巣癌（再発、進行期で化学療法抵抗性もしくは治療不耐性）
陰茎癌（再発、進行期）
副腎癌　（再発、進行期）　
悪性中皮腫（再発、進行期）　
小腸癌（再発、進行期で化学療法抵抗性もしくは治療不耐性）
メルケル細胞癌（再発、進行期）　
原発性皮膚悪性リンパ腫（再発、進行期）
2 標準的治療法では有効性が期待できない、もしくは標準的治療法を拒否した患者
3 1）の腫瘍においてWT1遺伝子が発現されていることが証明されている患者。WT1遺伝子の発現については免疫染色法により解析し判定する。
4 HLA型がHLA-A-24:02またはHLA-A-02:01である患者
5 測定可能な病変を有する患者
6 原発性脳腫瘍以外においては、原則としてCTまたはMRIによる評価で中枢神経系の病変が無いもしくは十分コントロールされている、あるいは急速な進行はないと考えられる患者
7 16歳以上85歳以下の患者
8 ECOG Performance Status (PS) が0-1の患者。ただし、原発性脳腫瘍においては、腫瘍によって引き起こされる神経症状のためにPS 2に低下している患者は可とする。
9 主要臓器機能の機能が保持されている患者
10 胸水・腹水・心嚢水が無い、もしくは、十分にコントロールされている患者
11 6ヶ月以上の生存が期待されている患者
12 本試験参加について文書による同意が得られた患者

1 A patient with a malignancy below that is diagnosed histologically.
Thymic cancer, Stage III, IV
Thymoma, Stage III, IV
Soft tissue sarcoma, recurrent or advanced
Malignant glioma, recurrent or advanced
salivary gland cancer, recurrent or advanced
Head and neck mucoepidermoid carcinoma, recurrent or advanced
Olfactory neuroblastoma, recurrent or advanced
Head and neck mucosal malignant melanoma, recurrent or advanced
Ophthalmic malignant melanoma, recurrent or advanced
Cancer of the external auditory canal, recurrent or advanced
testicular cancer, recurrent or advanced, and refractory, or treatment intolerant
cancer of penis, recurrent or advanced
adrenal cancer, recurrent or advanced
malignant mesothelioma, recurrent or advanced
cancer of the small intestine, recurrent or advanced, and refractory, or treatment intolerant
Merkel cell carcinoma, recurrent or advanced
Cutaneous lymphoma, recurrent or advanced
2 A patient with an advanced or refractory disease or who rejected a standard therapy
3 WT1 expression in the tumor as determined by immunohistochemistry
4 A patient with HLA-A-24:02 or HLA-A-02:01
5 A patient with measurable tumor or tumors
6 A patient without progressive metastatic brain tumors. * This should be confirmed by CT or MRI imaging.
7 Age; 16-85 years-old.
8 ECOG Performance Status, PS :0-1. PS 2 is allowed in case with primary brain tumor if it is ascribed to neural dysfunctions caused by brain tumor.
9 A patient without severe dysfunctions in his/her vital organs
10 A patient without uncontrolled pleural effusion, ascites, or cardiac effusion
11 Life longer than 6 months is expected.
12 Written informed consent is obtained.

除外基準/Key exclusion criteria

1) コントロール不良な感染症（活動性の結核を含む）を有する患者
2) 合併症のために本臨床試験への参加がふさわしくないと考えられる患者もしくは重大な医学的事象のために安全性が損なわれる可能性のある患者。
3) 重症の精神障害を有する患者。
4) 他の臨床試験に参加している患者。
5) その他、試験責任医師または試験分担医師が不適当と判断した患者。
