To clarify the concordance of tertiary lymphoid structure (TLS) evaluation (presence and maturity) using transbronchial cryobiopsy (TBCB) specimens with the gold standard surgical specimens in patients with non-small cell lung cancer, and to validate TBCB as a preoperative diagnostic method.

Tertiary lymphoid structures (TLS) in the tumor microenvironment are attracting attention as predictive factors for the efficacy of immune checkpoint inhibitors, but accurate evaluation has been difficult with conventional forceps biopsy due to specimen crushing[cite: 31]. This study uses transbronchial cryobiopsy (TBCB), which allows for larger specimen collection and superior preservation of tissue architecture, to validate the diagnostic validity of TLS evaluation by comparing it with surgical specimens from the same patient. The goal is to establish TBCB as a novel pre-operative biomarker assessment method.

Key inclusion criteria

Patients with non-small cell lung cancer who underwent transbronchial cryobiopsy (TBCB) for preoperative diagnosis at our hospital

Patients who subsequently underwent surgical resection of the primary lesion

Patients whose medical records and preserved pathological specimens (paraffin blocks) are available for analysis

Key exclusion criteria

Cases with insufficient specimen volume for pathological evaluation (presence and maturity of TLS)

Cases that received preoperative chemotherapy or radiotherapy

Other cases deemed inappropriate for this study by the principal investigator
