Objectives

Narrative objectives1

In this study, we will evaluate the effectiveness of three reminder methods in improving response rates through a randomized controlled trial (RCT) in the Chiba area of the "Japan Multi-Institutional Collaborative Cohort Study" (J-MICC Study):
(1) No reminder sent (control group)
(2) Reminder postcard sent
(3) Questionnaire redelivery

The purpose of this study is to examine strategies for improving collection rates in mail surveys targeting elderly Japanese populations, with the aim of enhancing evidence-based health policies and contributing to the development of international epidemiological research and public health policies.

Assessment

Primary outcomes

Response proportion: The response rate (number of submissions/number sent) will be calculated for each group.

Key secondary outcomes

Response acquisition cost: The average cost (including printing, postage, redelivery expenses, etc.) required to obtain one response will be calculated for each intervention method.
Response quality: The proportion of complete responses (percentage of responses with all items completed) will be compared.

Results

Response rates were 0.8% in the control group, 12% in the postcard group, and 36% in the re-sent group (p < 0.001 for all comparisons). Logistic regression confirmed both interventions significantly increased responses compared with no reminder, independent of participant characteristics. Cost per response was 872yen for postcards and 505yen for re-sent questionnaires, making the latter more cost-effective despite higher total cost.
