Urokinase has been in short supply since around February 2022 due to quality deterioration during the manufacturing process of the active pharmaceutical ingredient, and the shortage continues as of February 2024. We investigated whether patients are suffering from disadvantages due to the reduction in treatment options for parapneumonic pleural effusion.

Between January 2014 and January 2024, 125 patients who were admitted to the Department of Respiratory Medicine at our hospital due to complicated parapneumonic pleural effusion and treated with thoracic drainage were included in the study.

Following the shortage in urokinase supply, the prevalence of fibrinolytic therapy decreased from 63% to 16%, whereas that of thoracoscopic surgery increased from 27% to 53%. The time to thoracoscopic surgery and surgical duration were shortened by 4 days and 33 min, respectively. No impact on outcome, length of hospital stay, or hospital costs was detected.

We retrospectively analyzed data from 113 patients, who were admitted to our respiratory department with complicated parapneumonic effusion and underwent thoracic drainage between January 2014 and January 2024.In this cohort, 43% were high-risk patients (according to the RAPID score).
