The purpose of this study is to measure ACP engagement of hospitalized patients before and after an inpatient ACP intervention by health social work professionals and to estimate the effect of the intervention.

1. Inpatients assigned to a health and social services specialist
2. inpatients over 65 years old
3. Inpatients with whom ACP can be discussed.
4. Inpatients who have family members who can discuss ACP with them.
5. Inpatients who have a health care provider who can discuss ACP with them.
1. inpatients who have previously been administered ACP
2. inpatients who are deemed mentally unstable and undesirable for ACP intervention or questionnaire survey by a health and social services specialist
3. inpatients whose physical condition is not stable and for whom ACP is not advisable
4. inpatients with an expected length of stay of less than 12 weeks for which the health and social services specialist is expected to be involved.
