The purpose of this study is to estimate the improvement in Goal Attainment Scales of residents before and after trained welfare counselors, collaborating with multiple professions, in a geriatric health services facility implement ACP interventions with residents.

Male and Female
1. Residents and their families who are assigned to a welfare counselor
2. Residents who are 65 years of age or older and their families who are 18 years of age or older
3. Residents and their families who are able to have a communication about ACP.
4. Residents and their families who have a medical provider who can discuss ACP with them.
1. Residents and their families previously conducting ACP
2. Residents and their families whose a welfare counselor judged that they are mentally unstable and that ACP intervention or questionnaire survey would be undesirable.
3. Residents and their families whose physical condition has not been settled and for whom ACP is not desirable.
4. Residents and their families who are estimated to be under 12 weeks of a welfare counselor's intervention.
