To investigate the prevalence of cervical injuries among hospitalised patients due to near-hanging

To investigate the incidence of near-hanging associated cervical injuries which include laryngeal fracture, laryngeal deviation, spinal cord injury, cervical spine fracture, and blunt cerebrovascular injury.
The primary outcome of this study is prevalence of a hanging-related cervical injury. We define hanging-related cervical injury as laryngeal fracture; laryngeal deviation; spinal cord injury; cervical spine fracture; or blunt cerebrovascular injury.
The secondary outcomes are prevalence of laryngeal fracture, laryngeal deviation, spinal cord injury, spinal concussion, cervical spine fracture, blunt cerebrovascular injury, hypoxic encephalopathy, and negative-pressure pulmonary edema, respectively.

Male and Female
All consecutive patients aged 18 years old or older who were hospitalised to the department of Emergency Medicine of the NHO Tokyo Medical Center due to near-hanging between April 2014 and June 2023.
Patients who had suffered cardiac arrest due to hanging and had been never resuscitated at the emergency department are excluded. We will also exclude the patients who had fatal injuries due to suicide means other than hanging.

The median age of the patients was 42 years (interquartile range (IQR) 31 to 62), 33 (52%) were women, 37 (59%) had psychiatric dis-eases, and 20 (32%) had a history of attempted suicide. Among all included patients, 14 (22%) per-formed hanging from a height above their own, 14 (22%) attempted drug overdose simultaneously, and 25 (40%) had cardiac arrest on the scene. At admission, the median GCS score was 6 (IQR 3 to 14), and 8 (13%) patients complained of neck symptoms. On examination, there was a ligature mark and neck tenderness in 46 (73%) and 2 (3%) patients, respectively.
During the study period, a total of 162 patients were admitted to our ED due to hanging. Of those, 99 patients were excluded for the following reasons: 97 patients for cardiac arrest that did not achieve a return of spontaneous circulation, one for an age younger than 18 years, or one for a fatal abdominal injury caused by stabbing for a suicide attempt. Thus, the remaining 63 patients were included in the final analysis.

A retrospective, single-center, observational study will be conducted to investigate the prevalence of cervical injuries among hospitalised patients due to near-hanging between April 2014 and June 2023 by using the electronic medical records. The primary outcome is prevalence of a hanging-related cervical injury, which includes laryngeal fractures, laryngeal deviations, spinal cord injuries, cervical spine fractures, and blunt cerebrovascular injuries. The primary outcome will be determined among patients with normal consciousness and no abnormal neck symptoms and signs at admission.
