(1) Intraoperative factors including surgical blood loss, duration of operation and intraoperative red blood cell transfusion, (2) postoperative hemorrhagic complications, (3) postoperative thromboembolic complications.

Key inclusion criteria

Participants are included if all following criteria are met: (1) Patients receiving antithrombotic therapy (antiplatelet therapy and/or anticoagulation therapy), (2) Those undergoing elective GE surgery (including open surgery, laparoscopic surgery, and robot-assisted surgery), and (3) Those being able to submit written consent before the study.

Key exclusion criteria

Exclusion criteria were as follows: (1) urgent surgery, (2) being difficult to obtain consent for explanations, such as consciousness levels and examples of cognitive decline, and (3) written consent being not obtained. The number of such patients is reported separately.

Results

Multivariate investigation of the entire cohort revealed that discontinuation of APT and chronic anticoagulant use were independent risk factors for postoperative thromboembolism. The post-matching evaluation demonstrated that the rates of thromboembolic complications were significantly different between the continued and discontinued APT groups (0.6% vs. 3.3%; P=0.012).

Baseline Characteristics

This prospective multicenter cohort study on the Safety and Feasibility of Gastroenterological Surgery in Patients Undergoing Antithrombotic Therapy (GSATT study) conducted at 14 clinical centers enrolled and screened patients between October 2019 and December 2021.

Participant flow

We screened 1175 patients between October 2019 and December 2021 at 14 clinical sites in Japan. Of these patients, the primary survey included 1170 (99.6%) patients.

None

Outcome measures

The primary outcomes were thromboembolic and bleeding complications.
