Anterior decompression with fusion (ADF) and posterior decompression with laminoplasty (LAMP) are both effective surgical procedures for cervical spondylotic myelopathy (CSM). Previously we reported that ADF provided superior neurologic outcomes to LAMP at 5 years after operation. In this study, we update the data to assess long-term clinical and radiologic outcomes.

Clinical outcomes were evaluated by comparing the recovery rate of the Japanese Orthopaedic Association (JOA) score between the two groups. Radiographs were taken before surgery and every year after surgery. Sagittal alignment of the C2-7 lordotic angle and range of motion (ROM) in flexion and extension were measured before and after surgery.

Cervical spondylotic myelopathy
Patients with myelopathy caused by cervical disc herniation and ossification of the posterior longitudinal ligament (OPLL), those with radiculopathy, and those with a history of previous cervical spine surgery or injury
