sofosbuvir and ribavirin
sofosbuvir 400mg once daily, and ribavirin twice daily (600 mg daily in patients with a body weight of 60 kg and under, 800 mg daily in patients weighing from 60 kg to 80 kg and under, and 1000 mg daily in patients with a body weight over 80 kg), orally for 12 weeks
