A population of 2,130 patients with type 2 diabetes mellitus who underwent consecutive treatments in the Department of Diabetes, Metabolism and Kidney Disease of Edogawa Hospital, Tokyo, Japan between 2009 and 2012 were screened for this study. In order to evaluate sufficiently the clinical characteristics of the patients, 401 subjects whose treatment periods in our department were less than six months and 293 subjects whose duration of diabetes was unclear based on their medical records were excluded from the current study. Thus, the associations between the duration of diabetes and the other clinical characteristics of the patients were examined in 1,436 subjects in the present cross-sectional study.

As the duration of diabetes increased, the age, insulin therapy and HbA1c increased in both the elderly and non-elderly groups, whereas the urinary CPR and eGFR decreased. The duration of diabetes was significantly associated with the prevalence of diabetic retinopathy, nephropathy and neuropathy, but not with CVD, CHD or PAD in the elderly patients. The duration of diabetes showed a significant association with the prevalence of both diabetic micro- and macroangiopathy in the non-elderly patients.

In the present cross-sectional study, we aimed to clarify the relationships between the duration of diabetes and the current statuses of 1) diabetic vascular complications, 2) the treatment for diabetes and 3) risk factors for atherosclerotic disease in elderly patients with type 2 diabetes in comparison with those in non-elderly subjects.
