To investigate both conventional and collagen staining of nodular lesions in patients with biopsy-proven DN, and to compare the renal prognosis between two groups classified according to the staining pattern of nodular lesions.

Among patients with diabetes who underwent renal biopsy at Toranomon hospital from March 1985 to March 2013 and were confirmed to have diabetic nephropathy, those with glomerular class III (nodular sclerosis) according to the Renal Pathology Society classification were eligible and enrolled.
Patients whose baseline eGFR was <15 mL/min/1.73m2

In all patients, staining of nodular lesions with periodic acid Schiff (PAS), periodic acid methenamine silver (PAM), Masson trichrome, and collagens I, III, IV, V, and VI was performed using frozen section at the time of renal biopsy.
The Cox proportional hazard model was used to calculate the hazard ratio (HR) and 95% confidence interval (CI) for the death-censored endpoint. We investigated the hazard ratio of the difference in staining for the endpoint.
