To elucidate the effect of postprandial glucose lowering by miglitol on blood pressure and heart rate variability in heart failure or acute myocardial infarction patients with type 2 diabetes.
Efficacy
1. Change in circadian blood glucose levels assessed by continuous glucose monitoring (CGM)
2. Arrhythmia, heart rate variability, and T-wave alternans (TWA) assessed by Holter ECG monitoring.
1. Blood pressure and heart rate variability evaluated by ABPM.
Changes in circadian BP rhythm, BP level, and heart rate.

Male and Female
1. Stable HF patients with NYHA II or above
2. Patients with AMI who have finished rehabilitation, and just before leaving hospital.
3. HbA1c level 6.5% (NGSP) or more, or diagnosed as diabetes, or taking antdiabetic medications.
4. Age more than 20 yrs.
1. Patients with significant arrhythmia such as atrial fibrilation.
2. Patients with decompensated heart failure, fever, inflammation (CRP>1.0mg/dl)
3. Patients who are using insulin
4. Hepatic damage (AST, ALT >100 IU/L)
5. Patients with HBV, or HCV infection
6. Moderate renal dysfunction (serum creatinine >2.0mg/dl)
7. Patients with inflammatory bowel disease, colon ulcer, and bowel obstruction.
8. Severe chronic bowel diseases
9. Patients who have disease with increased bowel gas.
10. Patients with a history of gastric resection.
11. Lactose intolerance
12. Pregnant women or possible pregnancy.
13. Contraindication for miglitol
14. Dementia from whom we cannot obtain informed consent.
15. For whom the attending doctor considered he or she is not appropriate for the study.
