1. definite COPD
2. high risk group
a. exposure to tobacco or dusts + chronic cough, sputum, dyspnea
b. emphysema on chest CT
Key exclusion criteria
1. diffuse lung lesion confirmed by plain chest X-ray film
2. uncontrollable malignancy
