日本語
IgG4関連疾患患者を対象としたランダム化比較試験を系統的に評価し、糖皮質ステロイドを基本とする治療に免疫調節療法を追加した場合の有効性と安全性を、追加免疫調節療法を行わない対応する糖皮質ステロイド治療または同一の糖皮質ステロイド減量療法を併用したプラセボと比較する。疾患再燃、寛解、有害事象および糖皮質ステロイド節約効果を評価し、治療の種類、背景の糖皮質ステロイド投与法およびアウトカム定義の違いを考慮する。

To synthesize randomized trials comparing immunomodulatory therapies added to glucocorticoid-based treatment with corresponding glucocorticoid-based treatment alone or placebo with the same taper in IgG4-related disease. Efficacy, safety and glucocorticoid-sparing effects will be assessed, considering differences in therapies, glucocorticoid regimens and outcome definitions.

治療開始後約12か月（52週）における、各原試験で定義されたIgG4関連疾患の再発または再燃を経験した患者の割合。再発／再燃の定義および背景の糖皮質ステロイド投与法の違いを考慮して解析する。

Proportion of patients experiencing trial-defined IgG4-related disease relapse or flare, primarily at approximately 12 months (52 weeks) after treatment initiation. Differences in relapse/flare definitions and background glucocorticoid regimens will be considered in the analysis.

副次アウトカム評価項目/Key secondary outcomes

各試験で定義された寛解または完全寛解の割合（主に12か月／52週）、少なくとも1件の有害事象を経験した患者の割合、重篤な有害事象を経験した患者の割合、初回再発／再燃までの時間、年換算再燃率、および52週までの累積救援糖皮質ステロイド投与量。安全性の評価期間は各試験の追跡期間とする。異なる定義の寛解および総糖皮質ステロイド投与量と救援投与量は区別し、比較可能なデータのみを統合する。

Trial-defined remission or complete remission, primarily at approximately 12 months (52 weeks); proportion of patients with at least one adverse event; proportion with serious adverse events; time to first relapse/flare; annualized flare rate; and cumulative rescue glucocorticoid dose through week 52. Safety outcomes will use each trial's follow-up period. Remission definitions and total versus rescue glucocorticoid doses will be distinguished, and only comparable data will be pooled.

IgG4関連疾患患者を対象とし、糖皮質ステロイドを基本とする治療への免疫調節療法の追加と、追加療法を行わない対応する糖皮質ステロイド治療または同じ減量療法を併用するプラセボを比較したランダム化比較試験。疾患再発／再燃、寛解または安全性について評価可能なデータを報告する研究を対象とする。年齢および性別による追加の制限は設けない。

Randomized controlled trials in patients with IgG4-related disease comparing added immunomodulatory therapy plus glucocorticoid-based treatment with corresponding glucocorticoid-based treatment alone or placebo with the same taper, reporting evaluable relapse/flare, remission or safety data. No additional age or sex restriction will be imposed.

除外基準/Key exclusion criteria

日本語
非ランダム化研究、観察研究、症例報告、総説、重複報告、および研究の比較条件に合致しない試験。糖皮質ステロイド維持療法と中止のみを比較し、免疫調節療法の追加を評価しない試験は除外する。同じ試験の複数報告は統合し、二重計上を避ける。

Nonrandomized studies, observational studies, case reports, reviews, duplicate data and trials not matching the specified comparison will be excluded. Trials comparing glucocorticoid maintenance with withdrawal without evaluating added immunomodulatory therapy will be excluded. Multiple reports of the same trial will be linked to avoid double counting.

IgG4関連疾患における糖皮質ステロイドを基本とする治療への免疫調節療法の追加を評価するランダム化比較試験のシステマティックレビューとメタアナリシス。文献検索の対象期間は2026年9月30日までとする。現在は予備的検索のみで、正式な文献選別、データ抽出および定量的統合は未開始。比較可能なアウトカムについてランダム効果モデルで統合し、治療の種類およびアウトカム定義を考慮して解析する。新たな参加者の募集や個別患者データの収集を行わず、公表された集計データを用いるため倫理審査は該当しない。フォロー終了予定日は本レビューの完了予定日を示す。

Systematic review and meta-analysis of randomized trials of add-on immunomodulatory therapy with glucocorticoid-based treatment for IgG4-RD. Searches cover records through 30 September 2026. Literature searching is ongoing; quantitative synthesis is incomplete. Comparable outcomes will be pooled using random-effects models, considering therapy class and outcome definitions. No new participants or individual patient data are collected; published aggregate data are used and ethics review is not applicable.
