本研究の目的は、国内の急性期病院1施設に勤務する看護師を対象に、デスカフェ介入がターミナルケア態度に及ぼす影響をランダム化比較試験により検証することである。あわせて、臨老式死生観尺度の各下位尺度得点の変化、およびその変化とターミナルケア態度の変化との関連を探索的に検討する。さらに、介入後のフォーカスグループインタビューおよび質問紙の自由記載を用いて、参加者の対話体験や気づきを明らかにし、量的結果の背景や意味を補足的に理解する。

The primary objective of this study is to evaluate the effects of a Death Cafe intervention on terminal care attitudes among nurses working at a single acute care hospital in Japan through a randomized controlled trial. Secondary exploratory objectives are to examine changes in each subscale score of the Death Attitude Inventory (DAI) and associations between these changes and changes in terminal care attitudes. In addition, post-intervention focus group interviews and free-text questionnaire responses will be used to explore participants' experiences of dialogue and insights, providing a complementary understanding of the context and meaning of the quantitative findings.

介入群(A群):
参加者主体で死や生について語り合うデスカフェに、1人につき1回、約60分参加する。1グループ5名を標準とし、必要に応じて4~6名で調整する。セッションは、導入・ルール説明5分、対話50分、結び5分で構成する。同一のファシリテーターが、事前に設定したテーマと進行チェックリストに基づいて進行する。
デスカフェ終了後、同日に、評価目的のフォーカスグループインタビューを1回、約60分実施する。デスカフェおよびインタビューの内容は録音する。
質問紙調査は、デスカフェの1週間前、1週間後および1か月後の計3回実施する。

Intervention group (Group A):
Each participant will attend one approximately 60-minute Death Cafe session involving participant-led dialogue about death and life. Each group will normally comprise five participants, with adjustments to four to six participants as needed. The session will consist of a five-minute introduction and explanation of ground rules, 50 minutes of dialogue, and a five-minute closing. The same facilitator will conduct all sessions using a predefined discussion theme and a facilitation checklist.
One approximately 60-minute focus group interview will be conducted for evaluation purposes on the same day after the Death Cafe. Both the Death Cafe and the interview will be audio-recorded.
Questionnaires will be administered one week before, one week after, and one month after the Death Cafe.

対照群(B群):
比較評価期間中は、研究によるデスカフェおよびフォーカスグループインタビューを実施せず、通常の業務・生活を継続する。
あらかじめ設定したデスカフェ予定日を基準として、その1週間前、1週間後および1か月後に、介入群と同様の質問紙調査を計3回実施する。
研究期間終了後、希望者にデスカフェへの参加機会を設ける。

Control group (Group B):
Participants will continue their usual work and daily activities without receiving a study-provided Death Cafe session or focus group interview during the comparative assessment period.
The same questionnaires as those used in the intervention group will be administered at three time points: one week before, one week after, and one month after a prespecified corresponding Death Cafe date.
After the study period ends, participants who wish to attend will be offered an opportunity to participate in a Death Cafe.
