本研究の目的は、総胆管結石および総胆管結石性胆管炎に対するERCP治療において、Green Endoscopyの観点からデバイス使用数削減を意識した介入を行い、治療成績を維持しながらデバイス使用数を削減できるかを検証することである。

The objective of this study is to verify whether the number of devices used during ERCP for common bile duct stones and associated cholangitis can be reduced-while maintaining treatment outcomes-by implementing interventions aimed at minimizing device usage from the perspective of Green Endoscopy.

ERCPを施行する術者を対象に、Green Endoscopyの概念、内視鏡診療の環境負荷およびデバイス使用数削減の具体的方策に関する教育セッションを実施する。教育後、総胆管結石または総胆管結石性胆管炎に対する同一入院中のすべてのERCPにおいて、以下の方策を臨床的に適切な範囲で実践する。
ESTナイフのカニュレーション機能を活用したカテーテル使用の削減。
ESTとEPBDまたはEPLBDの一体型デバイスの活用。
可能な場合、採石バスケットまたは採石バルーンのいずれか一方での結石除去。
可能な場合、最初に選択したデバイスでの治療完遂。
処置前の症例検討による計画的なデバイス選択。
デバイス選択の最終判断は各術者の裁量に委ね、治療の安全性と有効性を最優先とする。必要に応じてデバイスの追加・変更を行う。介入導入前の通常診療症例をヒストリカル対照として、デバイス使用数、治療成績および安全性を比較する。

Endoscopists performing ERCP receive an educational session covering Green Endoscopy principles, the environmental impact of endoscopic practice, and practical strategies to reduce device use. Following the session, the following strategies are implemented, when clinically appropriate, during all ERCP sessions within the same hospitalization for common bile duct stones or associated cholangitis:
Using the cannulation function of a sphincterotome to reduce the use of separate cannulation catheters.
Using integrated devices for endoscopic sphincterotomy and papillary balloon dilation, including large-balloon dilation.
Using either a stone extraction basket or an extraction balloon alone whenever feasible.
Completing treatment with the initially selected device whenever feasible.
Planning device selection through preprocedure case discussions.
Final device selection remains at the discretion of each endoscopist, with treatment safety and efficacy taking priority. Additional devices or device changes are permitted as clinically necessary. Device use, treatment outcomes, and safety are compared with historical controls treated under usual practice before implementation.

1. Patients diagnosed with common bile duct stones or cholangitis due to common bile duct stones.
2. Patients for whom stone removal by ERCP is indicated.
3. Patients aged 20 years or older.

1. Billroth-I法以外の術後再建腸管を有する患者。
2. 肝内胆管結石を主病変とする患者。
3. 悪性胆道狭窄を合併する患者。
4. 経皮経肝的アプローチが必要な患者。
5. 研究参加への文書同意が得られない患者(介入後群のみ)。

1. Patients with surgically altered gastrointestinal anatomy other than Billroth I reconstruction.
2. Patients whose primary disease is intrahepatic bile duct stones.
3. Patients with concomitant malignant biliary strictures.
4. Patients requiring a percutaneous transhepatic approach.
5. Patients from whom written informed consent for study participation cannot be obtained (post-intervention group only).
