目的1/Narrative objectives1

日本語
当院の高度治療室(HCU)入室中の患者に対して、せん妄予防を目的とした非薬理学的な介入を行う。
介入方法は、具体的には日中に患者との対話により、患者が選択した映像を短時間視聴することにより、覚醒を促すことによって日中のサーカディアンリズムを維持し、夜間の睡眠を確保することによって、せん妄予防を図る。この介入が有効であるかどうかを明らかにする。

We will implement non-pharmacological interventions aimed at preventing delirium for patients in our hospital's High Care Unit (HCU). Specifically, the intervention involves engaging patients in conversation during the day and having them watch videos of their choice to promote wakefulness; this aims to maintain the daytime circadian rhythm and ensure nighttime sleep, thereby preventing delirium. We will evaluate the efficacy of this intervention.

選択基準/Key inclusion criteria

以下の基準をすべて満たす患者を対象とする
1） 当院HCUに７日以上入室すると予測される患者
2） 同意取得時において年齢が20歳以上の患者
3） 性別不問
4） 本研究の参加にあたり十分な説明を受けた後、十分な理解の上、患者本人の自由意思による文書同意が得られた患者

Patients meeting all of the following criteria are eligible:
1) Patients expected to be admitted to the hospital's HCU for 7 days or longer
2) Patients aged 20 years or older at the time of obtaining consent
3) Gender: No restriction
4) Patients who have provided written consent of their own free will, based on a full understanding after receiving a thorough explanation regarding participation in this study

除外基準/Key exclusion criteria

以下のいずれかに抵触する患者は本研究に組み入れないこととする
1） 認知症の既往のある患者
2） 重度精神疾患の既往のある患者
3） HCU入室時にすでにせん妄状態にある患者
4） 中枢神経に器質的障害を有する患者
5） 昏睡状態が遷延されると予想される患者
6） 言語理解が著しく困難な患者
7） 終末期・緩和ケア中の患者

Patients meeting any of the following criteria shall be excluded from this study:
1) Patients with a history of dementia
2) Patients with a history of severe mental illness
3) Patients already in a state of delirium upon admission to the HCU
4) Patients with organic central nervous system disorders
5) Patients expected to remain in a prolonged coma
6) Patients with severe difficulty in language comprehension
7) Patients receiving end-of-life or palliative care
