ベンゾジアゼピン系薬剤の長期服用患者では、耐性などによりレミマゾラムの麻酔効果が不安定となり、必要投与量の増加や覚醒遅延を生じる可能性がある。また、フルマゼニルによる拮抗は、離脱症状や不穏、過換気などを誘発する可能性があり、その使用には注意を要する。
レミマゾラムとプロポフォールを併用するHybrid TIVAでは、作用機序の異なる2剤を組み合わせることで、それぞれの必要投与量および効果部位濃度を低減し、麻酔効果の遷延とフルマゼニルの使用を抑制できる可能性がある。
本研究の目的は、ベンゾジアゼピン系薬剤を長期服用している患者を対象として、レミマゾラムとプロポフォールを併用した全静脈麻酔における麻酔導入および維持の確実性、覚醒時間、フルマゼニル使用頻度ならびに周術期有害事象を前向きに評価し、本麻酔法の安全性および有効性を検証することである。

In patients receiving long-term benzodiazepine therapy, tolerance may lead to an unpredictable anesthetic response to remimazolam, potentially resulting in increased dose requirements and delayed emergence. Furthermore, reversal with flumazenil requires caution because it may precipitate withdrawal symptoms, agitation, or hyperventilation.
Hybrid total intravenous anesthesia (Hybrid TIVA), combining remimazolam and propofol, may reduce the required doses and effect-site concentrations of both agents. This approach may consequently minimize prolonged anesthetic effects and reduce the need for flumazenil.
This prospective study aims to evaluate the safety and effectiveness of Hybrid TIVA with remimazolam and propofol in patients receiving long-term benzodiazepine therapy. The study will assess the reliability of anesthetic induction and maintenance, time to emergence, frequency of flumazenil administration, and perioperative adverse events.

本研究は、ベンゾジアゼピン常用患者を対象とした単施設前向き観察研究である。通常診療としてレミマゾラムとプロポフォールを併用した全身麻酔を受ける患者を対象とし、十分な鎮静が得られるかを主要評価項目、術後不穏の有無を副次評価項目として評価する。麻酔方法は研究参加の有無とは独立して診療上の判断により決定される。

This is a single-center prospective observational study in patients who regularly use benzodiazepines and receive general anesthesia with combined remimazolam and propofol as part of routine clinical care. The primary outcome is whether adequate sedation is achieved. The secondary outcome is the presence or absence of postoperative agitation. The anesthetic technique is determined by clinical judgment independently of study participation.
