本研究の主目的は、脳梗塞または脳出血後に片麻痺に伴う病態失認を呈する患者を対象として、通常のリハビリテーションに直流前庭電気刺激を追加した場合の病態失認重症度に対する効果を探索的に評価することである。副次的に、半側空間無視、運動機能、日常生活活動および認知機能の変化、介入効果の持続性ならびに直流前庭電気刺激の実施可能性を評価する。

The primary objective of this study is to explore the effects of adding galvanic vestibular stimulation to usual rehabilitation on the severity of anosognosia for hemiplegia in patients with ischemic or hemorrhagic stroke. The secondary objectives are to evaluate changes in unilateral spatial neglect, motor function, activities of daily living, and cognitive function, as well as the persistence of the intervention effects and the feasibility of galvanic vestibular stimulation.

Scores or severity categories on Feinberg anosognosia questionnaire, the Bisiach anosognosia score, Catherine Bergego Scale total score, Fugl-Meyer Assessment upper- and lower-extremity scores, Functional Independence Measure score, and Mini-Mental State Examination score at baseline and at weeks 2, 4, and 6. Changes in the VATA-m severity score from baseline to week 2 and from week 4 to week 6.

通常のリハビリテーションに直流前庭電気刺激を追加するA-B-A′期構造の単群介入。研究期間は6週間である。A期(研究開始から2週間)は、下肢筋力強化、バランス練習および歩行練習等を含む通常のリハビリテーションのみを実施する。B期(3~4週目)は、通常のリハビリテーションに加え、複合型電気治療器Intelect Advance Combo(Chattanooga社、医療機器認証番号:220AIBZX00049000)を用いた直流前庭電気刺激を実施する。対象者を座位とし、左右の乳様突起部に3×3 cmの電極を貼付する。刺激時間は1回15分、原則として平日1日1回、週5回、2週間、合計10回とする。刺激強度は、介入前に測定した感覚閾値および対象者の忍容性に基づき、不快感のない範囲で個別に調整し、目安を1.5 mAとする。A′期(5~6週目)は、再び通常のリハビリテーションのみを実施する。

A single-arm intervention with an A-B-A-prime phase structure in which galvanic vestibular stimulation is added to usual rehabilitation. The study period is 6 weeks. During the A phase (weeks 1-2), participants receive usual rehabilitation alone, including lower-extremity muscle strengthening, balance training, and gait training. During the B phase (weeks 3-4), galvanic vestibular stimulation is added to usual rehabilitation using the Intelect Advance Combo combination electrotherapy system (Chattanooga; medical device certification number: 220AIBZX00049000). Participants receive stimulation in a seated position with 3 x 3 cm electrodes placed over the bilateral mastoid processes. Stimulation is delivered for 15 minutes once daily on weekdays, 5 days per week for 2 weeks, for a total of 10 sessions. The stimulation intensity is individually adjusted within a tolerable, non-discomfort range based on the sensory threshold measured before the intervention and participant tolerability; the target intensity is approximately 1.5 mA. During the A-prime phase (weeks 5-6), participants receive usual rehabilitation alone again.

2.脳梗塞または脳出血の診断を受け、片麻痺を呈する者
3.病態失認評価により、片麻痺に伴う病態失認を有すると判断される者
4.半側空間無視またはプッシャー症候群の少なくとも一方を呈する者
5.研究参加について、本人または適切な代諾者から文書による同意が得られた者

1.Patients admitted to the Convalescent Rehabilitation Ward of Kansai Electric Power Hospital
2.Patients diagnosed with ischemic or hemorrhagic stroke who present with hemiplegia
3.Patients judged to have anosognosia for hemiplegia based on an anosognosia assessment
4.Patients presenting with at least one of the following: unilateral spatial neglect or pusher syndrome
5.Patients for whom written informed consent to participate in the study is obtained from the participant or an appropriate legally authorized representative

1.Patients judged to have difficulty understanding the study procedures or cooperating with the study assessments because of severe dementia, aphasia, disorders of consciousness, or other conditions
2.Patients with implanted medical devices or metal materials that may interfere with study participation, such as a pacemaker, cochlear implant, or aneurysm clip, in the intracranial or cervical region
3.Patients with a history of epileptic seizures
4.Patients with skin disease or wounds at the mastoid processes, which are the stimulation sites
5.Patients whom the attending physician or principal investigator judges to be inappropriate for participation in the study for any other reason
