脆弱性骨折の初発となる橈骨遠位端骨折(手首の骨折)は高率で大腿骨骨折等の二次骨折が発生する.この発生メカニズムについて申請者らの研究結果を併せると,骨折後の固定による“上肢活動量左右対称性の変化”,および“動的バランス機能”が影響因子として挙げられた.また,先行研究において短期間の固定による“筋収縮効率性”および“運動想起能力”の低下が報告されている.しかし,これらの因果関係は不明である.本研究は,上肢の短期間固定による前述因子への影響を検討することで,二次骨折発生メカニズムの一端を明らかにすることを目的とする.本研究結果は,新規的介入法の開発に資することが期待される.

Distal radius fracture, a common initial fragility fracture, is associated with a high risk of subsequent fractures, including hip fractures. Based on our previous findings, changes in bilateral symmetry of upper-limb activity and dynamic balance function have been identified as potential factors contributing to secondary fracture risk. Previous studies have also reported that short-term immobilization may reduce muscle contraction efficiency and motor imagery ability. However, the causal relationships among these factors remain unclear. This study aims to investigate the effects of short-term upper-limb immobilization on these factors and thereby elucidate part of the mechanism underlying secondary fractures. The findings of this study are expected to contribute to the development of novel intervention strategies.

つまみ力維持課題における変動係数、最大握力、最大握力発揮時の手関節筋表面筋電図における中間周波数および最大振幅、ハンドメンタルローテーションテストにおける左右弁別反応時間、8-Foot Up-and-Go Test(8FtG)およびFour Square Step Test(FSST)の遂行時間、Kinesthetic and Visual Imagery Questionnaire(KVIQ)の各項目得点。これらを固定前および固定後に評価する。

Coefficient of variation during the pinch-force maintenance task; maximal grip strength; median frequency and peak amplitude of wrist muscle surface electromyography during maximal grip exertion; reaction time for left-right discrimination in the Hand Mental Rotation Test; completion times for the 8-Foot Up-and-Go Test (8FtG) and Four Square Step Test (FSST); and scores for each item of the Kinesthetic and Visual Imagery Questionnaire (KVIQ). These outcomes will be assessed before and after immobilization.

18歳以上50歳未満の健常成人で、エジンバラ利き手テストの利き手指数が90%以上の右利き者。上肢機能、運動イメージおよびバランス機能に影響を及ぼす整形外科疾患または中枢神経疾患等の既往がなく、研究参加について文書による同意が得られた者。

Healthy adults aged 18 years or older and younger than 50 years who are determined to be right-handed based on a laterality quotient of 90% or higher on the Edinburgh Handedness Inventory. Participants must have no history of orthopedic or central nervous system disorders that could affect upper limb function, motor imagery, or balance function, and must provide written informed consent to participate in the study.
