目的/Objectives

日本語
胃癌に対する胃切除術後の遷延性入院期間を予測する機械学習モデルを開発し、内部交差検証および時間的妥当性検証(開発コホート1996-2015年、検証コホート2016-2026年)により、その予測性能・較正・臨床的有用性を評価する。また、DPC制度に基づくアウトカム定義がモデル性能に与える影響を検討する。

To develop machine learning models predicting prolonged postoperative stay after gastrectomy for gastric cancer, and to evaluate their discrimination, calibration, and clinical utility using internal cross-validation and temporal validation (development cohort 1996-2015, test cohort 2016-2026). To additionally examine how DPC (Diagnosis Procedure Combination)-based outcome definitions affect model performance.

1996年4月から2026年3月までに亀田総合病院において胃癌に対して幽門側胃切除術、噴門側胃切除術、または胃全摘術を施行され、術後在院日数が記録されている患者

本研究はDPC(診断群分類包括評価)導入前後にわたる長期コホートを対象とし、固定アウトカム(術後14日目以降退院)に加えて、DPC入院期間IIに基づく患者別アウトカムを用いた感度分析を実施している。

This study includes a long-term cohort spanning periods before and after DPC (Diagnosis Procedure Combination) implementation, and incorporates a sensitivity analysis using a patient-specific outcome based on DPC Period II thresholds, in addition to the primary fixed outcome (discharge on or after postoperative day 14).
