本研究の目的は、腹腔鏡下鼠径ヘルニア修復術（TAPP：transabdominal preperitoneal repair）におけるカメラ用ポートの挿入部位として、臍切開と臍上切開の臨床的有用性を比較検討し、最適な手技の確立に寄与することである。

The objective of this study is to compare the clinical utility of transumbilical and supraumbilical incisions for camera-port placement in transabdominal preperitoneal repair (TAPP) and to contribute to establishing the optimal surgical technique.

主要アウトカム評価項目/Primary outcomes

日本語
術後1日目（術後24±6時間）におけるカメラ用ポート挿入部の創部疼痛。患者自身がVisual Analog Scale（VAS：0＝痛みなし、10＝想像しうる最悪の痛み）を用いて評価する。

Wound pain at the camera-port insertion site on postoperative day 1 (24 +/- 6 hours after surgery), assessed by the patient using a visual analog scale (VAS), ranging from 0 (no pain) to 10 (worst imaginable pain).

副次アウトカム評価項目/Key secondary outcomes

2．術後1週間および1カ月における手術部位感染（SSI）の発生
3．術前、術後1カ月および1年におけるQOL（EQ-5D-5L）
4．術後1年における創部の整容的満足度（Body Image Scale）
5．術後1年におけるカメラ用ポート挿入部の腹壁瘢痕ヘルニア発生（腹部CTで評価）
6．術後3時間、術後1週間および術後1カ月におけるカメラ用ポート挿入部の創部疼痛（VAS）ならびに鎮痛薬の使用回数

1. Operative time, defined as the time from the start to the end of surgery.
2. Incidence of surgical site infection (SSI) at 1 week and 1 month after surgery.
3. Quality of life assessed using the EQ-5D-5L before surgery and at 1 month and 1 year after surgery.
4. Cosmetic satisfaction assessed using the Body Image Scale at 1 year after surgery.
5. Incidence of incisional hernia at the camera-port insertion site at 1 year after surgery, assessed by abdominal computed tomography.
6. Wound pain at the camera-port insertion site at 3 hours, 1 week, and 1 month after surgery, assessed using the visual analog scale (VAS), and the number of analgesic doses used.

Transumbilical incision group: During laparoscopic inguinal hernia repair using the transabdominal preperitoneal approach, a skin incision is made within the umbilical depression, and a 10-mm camera port is inserted into the abdominal cavity using an open technique.

Supraumbilical incision group: During laparoscopic inguinal hernia repair using the transabdominal preperitoneal approach, a transverse skin incision is made above the umbilical depression, and a 10-mm camera port is inserted into the abdominal cavity using an open technique.

1．20歳以上の患者
2．臨床所見または画像検査により鼠径ヘルニアと診断された患者
3．腹腔鏡下鼠径ヘルニア修復術（TAPP）の適応と判断された患者
4．本研究への参加について、登録前に本人から文書による同意が得られた患者

1. Patients aged 20 years or older.
2. Patients diagnosed with inguinal hernia based on clinical findings or imaging examinations.
3. Patients considered eligible for laparoscopic inguinal hernia repair using the transabdominal preperitoneal approach.
4. Patients who provide written informed consent before enrollment in the study.

除外基準/Key exclusion criteria

日本語
1．予定切開部位または腹腔内に感染がある、もしくは感染が強く疑われる患者
2．再発鼠径ヘルニアの患者
3．両側鼠径ヘルニアの患者
4．嵌頓または穿孔などにより緊急手術を必要とする患者
5．臍または臍周囲に開腹手術痕がある患者
6．臍ヘルニアを併発している患者
7．透析を必要とする腎機能障害のある患者
8．妊娠中または妊娠している可能性がある患者
9．その他、担当医が本研究への参加を不適当と判断した患者

1. Patients with confirmed or strongly suspected infection at the planned incision site or within the abdominal cavity.
2. Patients with recurrent inguinal hernia.
3. Patients with bilateral inguinal hernia.
4. Patients requiring emergency surgery because of incarceration, perforation, or other conditions.
5. Patients with a previous laparotomy scar at or around the umbilicus.
6. Patients with concomitant umbilical hernia.
7. Patients with renal dysfunction requiring dialysis.
8. Patients who are pregnant or may be pregnant.
9. Patients considered unsuitable for participation in this study by the attending physician.
