目的1/Narrative objectives1

日本語
トランスサイレチン型心アミロイドーシス（ATTR-CM）における病期進行に伴うapical sparing patternをはじめとした各評価パラメータの変化を明らかにすること。それにより、心アミロイドーシス患者の病期や予後の予測における心エコー図検査の有用性について明らかにすること。

This study aims to elucidate longitudinal changes in the apical sparing pattern and other echocardiographic parameters according to disease progression in patients with transthyretin cardiac amyloidosis (ATTR-CM), and to determine the utility of echocardiography for predicting disease stage and prognosis in patients with cardiac amyloidosis.

観察可能な症例に対して以下の項目を観察する
・患者背景：年齢、性別、身長、体重、BSA、BMI、NYHA機能分類、内服薬、併存疾患
・血液検査：NT-proBNP、Troponin I、eGFR、Cre、Alb、Hb値
・心電図：心房細動の有無、ペースメーカ植込の有無、QRS幅、PQ時間、低電位差の有無
・心エコー図検査所見：LV mass、relative wall thickness、LVEF、GLS、Basal LS、Mid LS、Apical LS、relative apical sparing ratio、stroke volume、cardiac index、E/A値、e、E/e'値、LAVI、LA strain、TAPSE、FAC、RV-FWLS、RA strain

The following parameters will be assessed in patients eligible for observation

Patient characteristics age sex height body weight body surface area BSA body mass index BMI New York Heart Association NYHA functional class medications and comorbidities

Laboratory findings N terminal pro B type natriuretic peptide NT proBNP troponin I estimated glomerular filtration rate eGFR creatinine Cre albumin Alb and hemoglobin Hb

Electrocardiographic findings presence of atrial fibrillation presence of an implanted pacemaker QRS duration PQ interval and presence of low voltage QRS complexes

Echocardiographic findings left ventricular LV mass relative wall thickness left ventricular ejection fraction LVEF global longitudinal strain GLS basal longitudinal strain Basal LS mid ventricular longitudinal strain Mid LS apical longitudinal strain Apical LS relative apical sparing ratio stroke volume cardiac index E A ratio early diastolic mitral annular velocity e E e ratio left atrial volume index LAVI left atrial strain tricuspid annular plane systolic excursion TAPSE fractional area change FAC right ventricular free wall longitudinal strain RV FWLS and right atrial strain
