本研究は、大分大学医学部附属病院神経内科を受診する主観的/客観的な認知機能障害の訴えのある40歳以上90歳以下の患者50例を対象に、前向き・ランダム化クロスオーバーデザインにより、紙&ペン版MoCA-JとMoCA Solo-Jを同一被験者に4週間(±7日)の間隔で両方実施し、両検査の合計スコアの一致度(CCC、RMSD等)を検証する。あわせて、各認知ドメイン別スコアの相関やシステムバイアス、アバターガイダンス機能が受検者のパフォーマンスに与える影響、ユーザビリティ(簡便性・負担感・独力での完走率等)を評価し、MoCA Solo-Jの日本におけるMCI(軽度認知障害)スクリーニングツールとしての臨床的妥当性・有用性を明らかにすることを目的とする。

This study will enroll 50 patients aged 40-90 years with subjective or objective cognitive complaints who visit the Department of Neurology at Oita University Hospital. Using a prospective, randomized crossover design, each participant will undergo both the paper-and-pencil MoCA-J and MoCA Solo-J, administered approximately 4 weeks apart (+/-7 days). The study will evaluate the agreement between the total scores of the two tests (using CCC, RMSD, etc.), the correlation of domain-specific scores, systematic bias, the effect of the avatar-guided self-administration feature on participant performance, and usability (ease of use, burden, completion rate without assistance, etc.). The overall aim is to establish the clinical validity and utility of MoCA Solo-J as a screening tool for mild cognitive impairment (MCI) in Japan.

1. Correlation between the two tests for each cognitive domain score (visuospatial/executive function, attention, language and naming, abstraction, memory, orientation), using Pearson's correlation coefficient

4) 研究参加に関する十分な説明を理解し、本人から文書による同意が得られる者

Patients visiting the Department of Neurology, Oita University Hospital, who meet all of the following criteria:
1) Patients with subjective or objective cognitive complaints (selected at the discretion of the physician)

4) Patients who are able to understand the explanation of the study and provide written informed consent by themselves

1) 半年以内に認知症治療薬を新たに処方された者(認知症治療薬を長期服用しており症状が安定している者は組入れ可)
2) 1ヶ月以内に認知症治療薬の開始または変更があった者
3) 重度の認知障害を有する者
4) 重度の精神疾患(統合失調症、双極性障害など)を有する者
5) アルコールまたは薬物依存を有する者
6) 重篤な頭部外傷の既往を有する者
7) 著しい視覚障害または聴覚障害を有する者
8) タブレット端末の使用が著しく困難な身体障害を有する者
9) レビー小体型認知症が疑われる者(認知機能の変動が大きいため)
10) その他、研究担当者が被験者として不適当と判断した者

Patients who meet any of the following criteria are excluded:
1) Patients newly prescribed anti-dementia drugs within the past 6 months (patients on long-term anti-dementia drug treatment with stable symptoms are eligible)
2) Patients whose anti-dementia medication was started or changed within the past 1 month
3) Patients with severe cognitive impairment
4) Patients with severe psychiatric disorders (such as schizophrenia or bipolar disorder)
5) Patients with alcohol or drug dependence
6) Patients with a history of serious head trauma
7) Patients with marked visual or hearing impairment
8) Patients with a physical disability that makes the use of a tablet device markedly difficult
9) Patients with suspected dementia with Lewy bodies (due to large fluctuations in cognitive function)
10) Patients judged to be inappropriate as study participants by the investigators for any other reason

前向き観察研究。対象者は同一被験者が紙&ペン版MoCA-JとMoCA Solo-Jの両方を、4週間(±7日)の間隔をあけて受検する。学習効果および実施順序の影響を相殺するため、同意取得後にコンピューターにより検査の実施順序を無作為に決定し、A群(紙版MoCA-J→MoCA Solo-J)またはB群(MoCA Solo-J→紙版MoCA-J)のいずれかの順序で実施する。いずれの検査も通常診療で用いられる認知機能スクリーニング検査であり、検査以外の医療行為の割付けは行わない。採点は、被験者を特定できないよう加工されたデータを用いて医師または心理士が管理者ポータル上で実施する。

Prospective observational study. Each participant undergoes both the paper-and-pencil MoCA-J and MoCA Solo-J, with an interval of 4 weeks (+/-7 days) between the two assessments. To offset learning effects and order effects, the order of the two assessments is determined at random by computer after informed consent is obtained: Arm A (paper MoCA-J first, then MoCA Solo-J) or Arm B (MoCA Solo-J first, then paper MoCA-J). Both assessments are cognitive screening tests used in routine clinical practice, and no medical intervention other than the assessments is allocated. Scoring is performed by a physician or psychologist through the administrator portal using de-identified data.
