介護士が認知症高齢者と会話する際に用いているコミュニケーション上の工夫やスキルを明らかにし、記述・整理することを目的とする。介護士に対する半構造化インタビューにより、会話時に心掛けている点や実践上の工夫を聞き取るとともに、介護士と認知症高齢者との自由会話場面を映像・音声および視線計測により記録する。これらを分析し、発話内容、発話のタイミング、話題展開、会話の終了方法、視線行動など、実際の介護場面で用いられているコミュニケーションスキルの特徴を探索的に明らかにする。

The aim of this study is to identify, describe, and organize the communication strategies and skills used by caregivers when conversing with older adults with dementia. Semi-structured interviews will be conducted with caregivers to explore the points they consider important and the practical strategies they use during conversations. In addition, free conversations between caregivers and older adults with dementia will be recorded using video, audio, and eye-tracking measures. These data will be analyzed to exploratively characterize communication skills used in actual caregiving settings, including verbal content, timing of utterances, topic development, ways of ending conversations, and gaze behavior.

選択基準/Key inclusion criteria

1. 研究実施施設に勤務し、認知症高齢者の介護に従事する介護士で、本研究への参加について本人から同意が得られた者。
2. 研究実施施設を利用する認知症高齢者で、研究で予定する範囲の会話が可能であり、本人または代諾者から研究参加への同意が得られた者。認知症高齢者の日常生活自立度Ⅰ～Ⅲ程度を参考とするが、施設職員が会話可能と判断した場合にはこれに限定しない。

1. Caregivers working at the participating facility who provide care for older adults with dementia and who provide informed consent to participate in the study.

2. Older adults with dementia using the participating facility who are able to engage in conversation to the extent required for the study and for whom consent to participate is obtained from the participant or a legally authorized representative. Independence level 1-3 according to the Japanese criteria for independence in daily living of older adults with dementia will be used as a reference. However, participants will not be excluded solely on this basis if facility staff familiar with the individual judge that the person is able to engage in the required conversation.

本研究は、介護士と認知症高齢者とのコミュニケーションを対象とした前向き・探索的・横断的観察研究である。研究実施施設に勤務する介護士および同施設を利用する認知症高齢者のうち、選択基準を満たし、研究参加への同意が得られた者を対象とする。対象者は無作為抽出ではなく、参加の意思が確認された者から選定する。認知症高齢者については、研究で予定する範囲の会話が可能であることを、本人の状態をよく知る施設職員と相談の上で判断する。

介護士に対して最大30分程度の半構造化インタビューを行い、認知症高齢者との会話において心掛けている点や実践上の工夫について聞き取る。また、介護士と認知症高齢者が1対1で自由に会話する場面を最大30分間記録する。会話内容や会話方法について研究者から特定の指示は行わない。

測定・記録項目は、介護士の年齢、性別、介護歴、介護スキルに関する情報、認知症高齢者の年齢、性別、認知症高齢者の日常生活自立度、半構造化インタビューの音声および文字起こしデータ、会話場面の映像・音声、ならびに視線解析システムにより取得した介護士の視線データである。これらのデータを質的・量的に分析し、介護士が認知症高齢者との会話で用いるコミュニケーション上の工夫、発話行動および視線行動の特徴を探索的に記述・整理する。

This is a prospective, exploratory, cross-sectional observational study of communication between caregivers and older adults with dementia.

Participants will include caregivers working at the participating facility and older adults with dementia using the facility who meet the eligibility criteria and provide consent to participate in the study. Participants will not be randomly sampled. Potential participants will be informed about the study at the participating facility, and those who express willingness to participate and meet the eligibility criteria will be enrolled. For older adults with dementia, their ability to engage in the conversations required for the study will be determined in consultation with facility staff who are familiar with their condition.

Caregivers will participate in semi-structured interviews lasting up to approximately 30 minutes to explore the points they consider important and the practical strategies they use when communicating with older adults with dementia. In addition, one-to-one free conversations between caregivers and older adults with dementia will be recorded for up to 30 minutes. The researchers will not specify particular conversation topics or conversational styles.

Data collected will include caregivers' age, sex, years of caregiving experience, and indicators of caregiving skills; older adults' age, sex, and level of independence in daily living for older adults with dementia; audio recordings and transcripts of the semi-structured interviews; video and audio recordings of the conversations; and caregivers' gaze data obtained using an eye-tracking system.

These data will be analyzed qualitatively and quantitatively to exploratively describe and organize communication strategies, verbal behaviors, and gaze behaviors used by caregivers when communicating with older adults with dementia.
