CTで総胆管結石を指摘できない総胆管結石の診断に関して初回検査（EUSとMRCP）の正診率を比較検討する。

To compare the diagnostic accuracy of the initial examination (EUS versus MRCP) for the diagnosis of common bile duct stones that cannot be identified on CT.

副次アウトカム評価項目/Key secondary outcomes

一次検査におけるEUSおよびMRCPの診断能（感度、特異度、陽性的中率、陰性的中率）を評価する。
二次検査におけるEUSおよびMRCPの総胆管結石検出率を評価する。
EUSおよびMRCPの有害事象発生率を評価する。

To evaluate the diagnostic performance (sensitivity, specificity, positive predictive value, and negative predictive value) of EUS and MRCP as the primary examination.

介入1(EUS群):
本群に割り付けられた症例は、初回検査としてEUSを実施する。総胆管結石の有無(認める場合はサイズと個数)を記録する。結石が疑われた場合はERCPを実施し、ERCP所見に基づいて最終診断を行う。結石を認めない場合は、二次検査としてMRCPを実施する。

Intervention1(EUS group):
Patients allocated to this group undergo EUS as the initial examination. The presence or absence of common bile duct stones (including size and number when present) is recorded. If stones are suspected, ERCP is performed and the final diagnosis is made based on ERCP findings. If no stones are detected, MRCP is performed as the secondary examination.

介入2(MRCP群):
本群に割り付けられた症例は、初回検査としてMRCPを実施する。総胆管結石の有無(認める場合はサイズと個数)を記録する。結石が疑われた場合はERCPを実施し、ERCP所見に基づいて最終診断を行う。結石を認めない場合は、二次検査としてEUSを実施する。

1. 同意取得時に18歳以上の症例
2. CTで明らかな総胆管結石を指摘できない症例
3. 急性胆管炎・胆嚢炎診療ガイドライン2018で急性胆管炎の診断基準において疑診・確診の症例
4. 急性胆管炎・胆嚢炎診療ガイドライン2018で急性胆管炎の重症度が軽症・中等症の症例
5. インフォームド・コンセントを取得した症例

英語
1. Patients aged 18 years or older at the time of obtaining consent
2. Patients in whom common bile duct stones cannot be clearly identified on CT
3. Patients with suspected or definite diagnosis of acute cholangitis according to the diagnostic criteria in the Tokyo Guidelines 2018 (Guidelines for the management of acute cholangitis and cholecystitis)
4. Patients with mild or moderate acute cholangitis according to the severity grading in the Tokyo Guidelines 2018 (Guidelines for the management of acute cholangitis and cholecystitis)
5. Patients from whom informed consent has been obtained

除外基準/Key exclusion criteria

1. 妊娠中または妊娠の可能性がある、または授乳婦
2. 内視鏡（EUS）の施行が困難な症例
3. 閉所恐怖症・体内金属のためMRI検査困難な症例
4. ECOG Performance status 4の症例
5. ASA4以上の重篤な併存疾患（心疾患、肺疾患、腎疾患、肝疾患等）を有する症例
6. B-1再建以外の術後再建腸管の症例
7. 胆石性膵炎の症例
8. 胆管狭窄を併発している症例
9. 医師の判断により対象として不適当と判断された症例

1. Patients who are pregnant, possibly pregnant, or breastfeeding
2. Patients in whom endoscopy (EUS) is difficult to perform
3. Patients in whom MRI is difficult to perform due to claustrophobia or metal implants in the body
4. Patients with ECOG Performance Status 4
5. Patients with severe comorbidities of ASA class 4 or higher (cardiac, pulmonary, renal, hepatic disease, etc.)
6. Patients with surgically altered gastrointestinal anatomy other than Billroth-I reconstruction
7. Patients with gallstone pancreatitis
8. Patients with concomitant bile duct stricture
9. Patients judged to be inappropriate for enrollment at the physician's discretion
