片側人工股関節全置換術後患者において、免荷式歩行器の前進負荷機能を用いた歩行前・歩行中・歩行後の各条件で歩行時股関節伸展角度の変化を検討することを目的とする。

The purpose of this study is to investigate changes in hip extension angle during gait before, during, and after forward-resistance walking using a body weight support walker in patients after unilateral total hip arthroplasty.

主要アウトカム評価項目/Primary outcomes

歩行時の術側股関節最大伸展角度を以下の3条件で評価する：①免荷式歩行器の前進負荷機能を用いる前の通常歩行、②免荷式歩行器の前進負荷機能を用いた歩行、③免荷式歩行器の前進負荷機能を用いた歩行直後の通常歩行。

Maximum hip extension angle of the operated side during gait is assessed under the following three conditions: (1) usual walking before forward-resistance walking, (2) forward-resistance walking using a body weight support walker, and (3) usual walking immediately after forward-resistance walking.

副次アウトカム評価項目/Key secondary outcomes

歩行速度および歩幅（主要アウトカムと同じ3条件で評価）
免荷式歩行器の前進負荷機能を用いた歩行の自覚的運動強度（修正版Borg Scale、前進負荷歩行直後に評価）
歩行時の術側股関節痛（Numerical Rating Scale：NRS、主要アウトカムと同じ3条件で評価）
術側股関節の他動伸展可動域（歩行測定前に評価）

Rating of perceived exertion during forward-resistance walking using a body weight support walker (Modified Borg Scale; assessed immediately after forward-resistance walking).
Pain in the operated hip during gait (Numerical Rating Scale: NRS; assessed under the same three conditions as the primary outcome).

介入は1日で実施する。免荷式歩行器の前進負荷機能を用いた歩行を10 m×3回実施する。免荷式歩行器の免荷機能は使用しない。前進負荷機能は前進負荷の有無を切り替える機能であり、負荷量を任意に調整することはできない。免荷式歩行器の前進負荷機能を用いた歩行の前に通常歩行を10 m×3回実施し、前進負荷歩行直後に再度通常歩行を10 m×3回実施する。

The intervention is conducted on a single day. Participants perform three 10-m trials of forward-resistance walking using a body weight support walker. The body weight support function of the walker is not used. The forward-resistance function can be switched on or off, but the amount of resistance cannot be adjusted. Before forward-resistance walking using the body weight support walker, participants perform three 10-m trials of usual walking, followed by another three 10-m trials of usual walking immediately after forward-resistance walking.

選択基準/Key inclusion criteria

日本語
1）医療法人真木会真木病院に入院中の術後1～6週以内の片側人工股関節全置換術後患者。
2）本研究の内容について十分な説明を受け、本人より文書による同意が得られた者。
3）杖歩行または独歩が可能な者。
4）主治医および担当理学療法士により、本研究で実施する歩行課題を安全に遂行可能と判断された者。

英語
1.Patients hospitalized at Maki Hospital who are 1 to 6 weeks after unilateral total hip arthroplasty.
2.Individuals who have received a sufficient explanation of the study and provided written informed consent.
3.Individuals who are able to walk independently or with a cane.
4.Individuals who are judged by the attending physician and physical therapist to be able to safely perform the walking tasks required in this study.

除外基準/Key exclusion criteria

日本語
1）人工股関節全置換術術直後かつ歩行が安全にできない患者。
2）疼痛により本研究で実施する歩行距離の実施が困難な者。
3）非術側下肢の整形外科的疾患または神経学的疾患により明らかに歩行に影響を及ぼす者（例：人工関節置換術後、手術適応となる変形性関節症、大腿骨近位部骨折術後、脳卒中による片麻痺、パーキンソン病）。
4）認知機能低下または高次脳機能障害により研究内容の理解や指示理解が困難な者。
5）神経疾患、重篤な循環器疾患、呼吸器疾患などにより安全な歩行実施が困難と判断される者。
6）その他、主治医が研究参加を不適当と判断した者。
7）本研究で行う動画撮影に同意が得られない者。

英語
1.Patients in the immediate postoperative period after total hip arthroplasty who are unable to walk safely.
2.Individuals who are unable to complete the walking distance required in this study due to pain.
3.Individuals with an orthopedic or neurological condition of the non-operated lower limb that clearly affects gait (e.g., previous joint replacement, osteoarthritis requiring surgery, previous proximal femoral fracture, hemiplegia due to stroke, or Parkinson's disease).
4.Individuals who have difficulty understanding the study or following instructions due to cognitive impairment or higher brain dysfunction.
5.Individuals who are judged to be unable to walk safely due to neurological disease, severe cardiovascular disease, respiratory disease, or other conditions.
6.Individuals who are otherwise judged by the attending physician to be unsuitable for participation in the study.
7.Individuals who do not provide consent for video recording conducted in this study.
