目的/Objectives

目的1/Narrative objectives1

日本語
本研究の目的は、術前治療後に直腸癌手術および一時的人工肛門造設術を施行した患者を対象として、人工肛門閉鎖前後における肛門機能および排便機能障害の変化を前向きに評価することである。直腸肛門内圧検査、排便造影検査および質問票を用いて、人工肛門閉鎖前後の肛門機能、排便機能およびQOLの変化を明らかにすることを目的とする。

The aim of this study is to prospectively evaluate changes in anorectal function and defecatory dysfunction before and after temporary stoma closure in patients who have undergone rectal cancer surgery with temporary stoma creation following neoadjuvant therapy. Changes in objective anorectal function, bowel function, and quality of life before and after stoma closure will be assessed using anorectal manometry, defecography, and patient-reported questionnaires.

選択基準/Key inclusion criteria

以下の適格基準をすべて満たす症例を適格とする。

① 術前治療施行後に直腸癌に対して肛門温存直腸切除術を受け、その際に一時的回腸人工肛門造設術を施行された患者
② 直腸切除術後3か月から1年以内に人工肛門閉鎖を予定している患者
③ 大腸内視鏡検査にて吻合部に狭窄や潰瘍等の異常所見がないことが確認されている患者
④ 同意取得時の年齢が満20歳以上である患者
⑤ 研究参加者本人に説明文書を用いて説明を行い、本人から自由意思による文書同意が得られている患者

Patients who meet all of the following eligibility criteria will be considered eligible:

1. Patients who have undergone sphincter-preserving rectal resection with temporary ileostomy for rectal cancer following neoadjuvant therapy.
2. Patients scheduled for temporary stoma closure within 3 months to 1 year after rectal resection.
3. Patients confirmed by colonoscopy to have no abnormal findings, such as anastomotic stricture or ulceration.
4. Patients aged 20 years or older at the time of informed consent.
5. Patients who have provided written informed consent voluntarily after receiving an explanation of the study using the written informed consent document.

除外基準/Key exclusion criteria

以下のいずれかに該当する患者は除外する。

① 認知症や精神疾患等により、アンケートの実施および肛門機能検査を施行できない患者
② 人工肛門閉鎖を希望しない患者
③ バリウムや小麦粉に対するアレルギーを有する患者
④ その他、研究責任者が安全性の観点から本研究への参加が不適当と判断した患者

Patients meeting any of the following criteria will be excluded:

1. Patients who are unable to complete the questionnaires or undergo anorectal function testing due to dementia, psychiatric disorders, or other conditions.
2. Patients who do not wish to undergo temporary stoma closure.
3. Patients with an allergy to barium or wheat flour.
4. Patients whom the principal investigator considers unsuitable for participation in the study for safety reasons.
