The objective of this systematic review is to determine whether psychological support provided before the death of the patient to adult family members aged 18 years or older who are expected to lose a loved one in the near future due to cancer or another life threatening physical illness improves family outcomes, including anticipatory grief, psychological distress, anxiety, depression, caregiver burden, quality of life, and preparedness for death. The findings will be used to inform recommendations in a clinical practice guideline for family and bereavement care.

生前アウトカムについては、介入終了直後または患者の生存中に実施された介入後最初の評価時点を主要評価時点とする。複数の評価時点がある場合は、介入終了直後、短期および中長期に分けて評価する。複雑性悲嘆については、患者死亡後の各評価時点を対象とする。

The severity of psychological distress among adult family members or significant others aged 18 years or older who are expected to lose a loved one in the near future due to cancer or another physical illness, assessed while the patient is alive. Psychological distress includes general psychological distress, anticipatory grief, pre loss grief, anxiety, and related emotional distress. Scores after the intervention or changes from baseline measured using validated instruments will be evaluated.
For outcomes assessed before death, the primary time point will be immediately after completion of the intervention or the first assessment after the intervention conducted while the patient is alive. When multiple assessments are available, immediate, short term, and medium to long term outcomes will be evaluated separately.

The following secondary outcomes will be evaluated.
Quality of life. Scores after the intervention or changes from baseline measured using validated quality of life instruments.
Complicated grief or prolonged grief symptoms. Severity or incidence of grief symptoms assessed after the death of the patient using validated grief measures.
Depression. Severity of depressive symptoms measured using validated instruments, or incidence of depression as defined by each study.
Dropout rate. The proportion of participants allocated to psychological support who do not complete the intervention or study assessments.
Psychological burden associated with the intervention. Worsening of psychological distress, temporary worsening of anxiety or grief, or other psychological adverse events related to psychological support.
Economic burden. Direct or indirect costs incurred by family members, including intervention costs, transportation expenses, communication or equipment costs, and loss of income or work time.

本研究は、家族・遺族ケアに関する診療ガイドラインの推奨作成を目的としたシステマティックレビューである。がんまたはその他の身体疾患を有する患者を近い将来に失うことが予測される18歳以上の成人家族または重要他者を対象として、患者の生前に開始または実施される心理支援の有効性および安全性を検討する。
介入は、家族または重要他者の精神心理的苦痛の軽減を目的とする心理療法、カウンセリング、心理教育、心理社会的支援、コミュニケーション支援、オンラインまたはアプリを用いた心理支援等とする。対照は、通常ケア、介入なし、待機群、またはその他の対照介入とする。
主要アウトカムは精神心理的苦痛とし、全般的心理的苦痛、予期悲嘆、喪失前悲嘆、不安等を含む。副次アウトカムは、生活の質、複雑性悲嘆または遷延性悲嘆、抑うつ、ドロップアウト率、心理支援に伴う心理的負担、および経済的負担とする。
PubMedを含む研究計画書で定めた文献データベースを用いて系統的検索を行う。PubMedの検索日は2026年6月26日である。電子検索で把握できなかった関連研究については、引用文献検索およびハンドサーチを追加する。
文献の選択、データ抽出および研究の質の評価は、原則として複数の評価者が独立して実施し、意見の相違は協議によって解決する。無作為化比較試験およびその他の対照群を有する介入研究を対象とする。臨床的および方法論的に統合可能な研究についてはメタアナリシスを実施し、統合が困難な場合はナラティブに結果を要約する。研究間の異質性、バイアスのリスクおよびエビデンスの確実性を評価し、診療ガイドラインの推奨作成に用いる。

This systematic review will inform a clinical practice guideline for family and bereavement care. It will assess the effectiveness and safety of psychological support provided before the death of the patient to adult family members or significant others aged 18 years or older who are expected to lose a loved one in the near future due to cancer or another physical illness.
Eligible interventions include psychotherapy, counseling, psychoeducation, psychosocial support, communication support, and support delivered online or by mobile applications. Comparators include usual care, no intervention, waiting list, or another control intervention.
The primary outcome is psychological distress, including general distress, anticipatory grief, pre loss grief, and anxiety. Secondary outcomes include quality of life, complicated or prolonged grief, depression, dropout, psychological burden related to the intervention, and economic burden.
A systematic search will be conducted using PubMed and other databases specified in the protocol. The PubMed search was conducted on June 26, 2026. Citation searching and hand searching will also be performed.
Study selection, data extraction, and risk of bias assessment will be conducted independently by multiple reviewers. Randomized controlled trials and other controlled intervention studies will be included. Meta analysis will be performed when studies are sufficiently similar. Otherwise, findings will be summarized narratively. Heterogeneity and certainty of evidence will be assessed.
