介入群のプログラム内容は入院48時間以内に開始され，MI-Eに加え，排痰法（体位ドレナージ，咳嗽介助），早期離床を含む運動プログラムを全身状態，身体機能レベルに応じて，ベッド上運動より開始し，可能な場合は座位，立位，歩行へと段階的に内容を調整する．合わせて，下肢および体幹を中心とした筋力練習，ADL練習を5-6日/週実施する．排痰法およびMI-Eの終了基準は (1) 気管吸引の不要となる．(2) 気道分泌物を示唆する肺複雑音が消失する．(3) 酸素下が改善する．に該当する場合，総合的に判断する．早期離床を含む運動プログラムは入院期間中継続する．

The intervention program will be initiated within 48 hours of hospitalization. In addition to MI-E, airway clearance techniques (including postural drainage and cough assistance) and an exercise program will be provided. The exercise will be adjusted according to the patient's general condition and physical functional level, starting with bed-based exercises and progressing stepwise to sitting, standing, and walking when feasible. In addition, strengthening exercises mainly targeting the lower limbs and trunk, as well as activities of daily living (ADL) training, will be performed 5-6 days per week. The discontinuation criteria for airway clearance techniques and MI-E will be comprehensively determined when one or more of the following conditions are met: (1) airway suctioning is no longer required; (2) adventitious lung sounds suggestive of airway secretions disappear; and (3) oxygenation status improves. The exercise program including early mobilization will be continued throughout the hospitalization period.

The control intervention will be initiated within 48 hours of hospital admission. The program will include airway clearance techniques (postural drainage and assisted coughing) and an exercise program incorporating early mobilization. The exercise program will be individualized according to each participant's general medical condition and physical function, beginning with bed-based exercises and progressing, when feasible, to sitting, standing, and ambulation. In addition, lower-extremity and trunk muscle strengthening exercises, as well as activities of daily living (ADL) training, will be provided 5-6 days per week. Airway clearance techniques will be discontinued based on an overall clinical assessment when one or more of the following criteria are met: (1) airway suctioning is no longer required; (2) adventitious lung sounds suggestive of retained airway secretions have resolved; and/or (3) oxygenation has improved. The exercise program, including early mobilization, will be continued throughout the hospitalization.

選択基準/Key inclusion criteria

1. 入院 48 時間以内にリハビリの処方があった 65 歳以上の肺炎患者
2. 自力での咳嗽で痰の喀出が不十分もしくは不可能な場合
3. 気管吸引を必要とする．

1. Age sixty-five years or older. Diagnosis of pneumonia and prescription for rehabilitation within forty-eight hours of hospital admission.
2. Inadequate or absent sputum expectoration despite spontaneous coughing.
3. Requirement for tracheal suctioning.

除外基準/Key exclusion criteria

1. 気胸あるいはその既往，胸部画像上ブラを伴う慢性呼吸器疾患
2. 重度心疾患（NYHA 分類Ⅲ，Ⅳ）
3. 肋骨骨折や開胸術後など排痰法の実施が困難な合併症
4. ウイルス性肺炎
5. 研究の非同意
6. 人工呼吸管理

1. Current or prior pneumothorax, or chronic respiratory disease with pulmonary bullae on chest imaging.
2. Severe heart disease (New York Heart Association [NYHA] functional class III or IV).
3. Comorbidities precluding the application of airway clearance techniques, such as rib fractures or recent thoracotomy.
4. Viral pneumonia.
5. Refusal to provide informed consent for study participation.
6. Requirement for mechanical ventilation.
