我々は、術前手洗いおよびガウンテクニックなどの比較的手順が標準化された医行為において、臨床現場の経験を有する臨床実習生による指導は学修者の技能習得に寄与できる可能性があると考えた。このため、本研究では、臨床実習中の医学科5年生による臨床実習開始直前の医学科4年生に対する術前手洗いおよびガウンテクニック教育が、教員による教育に対して非劣勢であるかを検証することを目的とした。術前手洗いおよびガウンテクニックは、日本において、全ての医学生と初期臨床研修医が経験すべき医行為の一つであり、教育的価値が極めて高い。臨床実習生による技能教育の非劣性が示されれば、本医行為の教育におけるPALの正規カリキュラムへの導入、およびそれによる教員の負担軽減や医師の働き方改革の推進に寄与できる可能性があり、社会的意義が極めて高い。

We hypothesized that, for relatively standardized clinical procedures such as preoperative hand scrubbing and gowning techniques, instruction provided by clinical trainees with real-world clinical experience may contribute to learners' procedural skill acquisition. Therefore, this study aimed to evaluate whether instruction in preoperative hand scrubbing and gowning techniques provided by fifth-year medical students during their clinical clerkships was non-inferior to instruction provided by faculty members for fourth-year medical students immediately before the start of their clerkships. In Japan, preoperative hand scrubbing and gowning techniques are core clinical procedures that all medical students and residents are expected to perform and are considered to have substantial educational value. Demonstrating the non-inferiority of instruction provided by clinical trainees would support the integration of PAL into the formal curriculum for these procedures and may contribute to reducing faculty workload and promoting physician workstyle reforms, thereby providing substantial societal value.

対象者を、臨床実習中の医学科5年生から指導を受ける群（Student-as-Teachers：SAT群）と教員から指導を受ける群（教員群）の2群に割り付け、SAT群を介入群とした。全ての対象者は90分間の「術前手洗いおよびガウンテクニック実習」に参加し、指導を受けた。全ての指導者は、事前に標準的な手洗いおよびガウンテクニックについて統一された指導教材を共有され、それに基づいて指導をした。その指導教材には、介助者とのコミュニケーション、手洗い手順と範囲（合計3回）、滅菌タオルでのふき取り、アルコール製剤の使用、マスク付きガウンの受け取りと展開、ガウンの袖通しと着用、マスクの着用、腰ひもの受け渡し、滅菌手袋の受け取りと着用、およびガウンの脱衣が含まれた。

Participants were allocated to one of two groups: a group receiving instruction from fifth-year medical students during their clinical clerkships (SATs group) and a group receiving instruction from faculty members (faculty group). The SAT group served as the intervention group. All participants participated in the "preoperative hand scrubbing and gowning technique training session (90 minuites)" and instructed by each instructor. All instructors were provided with standardized instructional materials on preoperative hand scrubbing and gowning techniques before the training session. All instructions were delivered in accordance with these materials. The materials covered communication with the assistant; hand scrubbing procedures and the area (three cycles in total); drying scrubbed hands with sterile towels; application of an alcohol-based solution; receiving and unfolding a gown with an integrated mask; gowning procedures including receiving mask strings, sleeve insertion, mask application, and transfer of waist ties; receiving and donning sterile gloves; and gown removal.
