本研究は、腰椎のCT値が心臓手術中の輸血必要量の予測因子となり得るかどうかを後方視的に検討し、より精度の高い輸血必要量予測モデルの構築を目指す

2023年10月1日から2025年10月30日までに藤田医科大学岡崎医療センターで心臓手術（冠動脈バイパス術、弁置換・形成術）を受けた18歳以上の成人患者。大血管手術を除く。

Adult patients (over18 years) who underwent cardiac surgery (coronary artery bypass grafting, valve replacement/repair) at our institution between October 1, 2023, and October 30, 2025. Aortic surgery is excluded.

除外基準/Key exclusion criteria

再手術、術前に腰椎CTが施行されていない患者、入院中に他院へ転院した患者、診療録が不完全な患者、急性外傷や腫瘍等で骨構造に著明な異常を有する患者

Patients without preoperative lumbar CT; patients transferred to another hospital during admission; patients with incomplete medical records; patients with significant structural bone abnormalities due to acute trauma or tumors.

収集データ
患者背景：年齢、性別、BMI、基礎疾患（糖尿病、高血圧、腎機能障害等）、術前ヘモグロビン値、凝固能、アルブミン値
手術情報：手術の種類、手術時間、人工心肺時間、出血量、術中輸血量（赤血球、FFP、血小板等）、再手術の有無、術後出血量（胸骨下ドレーン、胸腔ドレーン）、術後輸血量
画像情報：術前に撮影された腰椎CT画像、（L3レベルの3か所の平均CT値［HU］）（2）、胸骨の3レベルの平均CT値
予後指標：術後合併症（感染症、腎不全、呼吸不全等）、ICU滞在日数、入院期間、死亡率

5-７-1. 主要評価項目
心臓手術中に必要となった総輸血量（ml、単位数）と腰椎CT値との関連
ドレーン留置中における術後１日当たりの出血量、並びに輸血量と腰椎CT値との関連

Patient characteristics: Age, sex, BMI, comorbidities (diabetes, hypertension, renal dysfunction), preoperative hemoglobin, coagulation profile, albumin level
Surgical information: Type of surgery, operative time, cardiopulmonary bypass time, intraoperative blood loss, intraoperative transfusion volume (RBC, FFP, platelets), reoperation, postoperative bleeding volume (mediastinal and thoracic drains), postoperative transfusion volume
Imaging data: Preoperative lumbar CT images; mean CT value (HU) at three points of the L3 level; mean CT values at three levels of the sternum
Outcome indicators: Postoperative complications (infection, renal failure, respiratory failure), ICU length of stay, hospital stay, mortality
