腹部大動脈瘤に対する人工血管置換術を施行する患者を対象に、術前Dual Energy CTによるMSU沈着評価と、手術摘出標本にHE染色およびDe Galantha染色を施したうえで偏光顕微鏡を用いて行う病理学的MSU同定を前向きに対比し、その一致率を検証するとともに局所炎症所見を解析する。

To prospectively compare preoperative dual-energy CT assessment of monosodium urate (MSU) crystal deposition with histopathological MSU identification performed by polarized light microscopy of hematoxylin-eosin (HE)-stained and De Galantha-stained sections from surgically resected specimens in patients undergoing open surgical repair for abdominal aortic aneurysm, and to evaluate concordance and local inflammatory findings.

(3) 本研究について十分な説明を受け、文書による同意が得られた患者

本研究は、腹部大動脈瘤に対する人工血管置換術を施行する患者を対象とした単施設前向き研究である。術前（手術6ヶ月前から手術前日）にDual Energy CT（DECT）を施行してMSU沈着を評価し、手術当日に摘出した大動脈瘤壁組織（1cm2から25cm2）をHE染色およびDe Galantha染色したうえで偏光顕微鏡（200から400倍）にて観察してMSU結晶を同定する。主要評価としてDECTと病理学的MSU同定の一致率をMcNemar検定で検証する。副次評価として血清尿酸値・高尿酸血症・痛風既往とMSU陽性率の関連、および局所炎症細胞浸潤とMSU陽性の関連を解析する。倫理委員会整理番号：第R-106-1号。

This is a single-center prospective study enrolling patients undergoing open surgical repair for abdominal aortic aneurysm. Preoperative dual-energy CT (DECT) is performed between 6 months before and the day before surgery to assess MSU crystal deposition. Aortic aneurysm wall tissue (1 to 25 cm2) harvested during surgery is stained with HE and De Galantha stain and examined under polarized light microscopy (200 to 400x magnification) to identify MSU crystals. The primary evaluation assesses concordance between DECT and histopathological MSU identification using the McNemar test. Secondary evaluations include the association between MSU positivity and serum uric acid levels, hyperuricemia, or history of gout, and the relationship between local inflammatory cell infiltration and MSU positivity. IRB approval number: R-106-1.
