除外基準/Key exclusion criteria

1) 入院時の体重欠測
2) 入院時の体重が10 kg未満または300 kg以上
3) 入院1～7日目に生存退院/死亡退院

メディカル・データ・ビジョン株式会社の診療データベース
本データベースはDPCデータ/診療報酬データをもとに構築されており、DPC対象病院の約32%をカバーしている。なお、全てのデータは個人情報保護の観点から匿名化処理されている。
(1) 急性期の栄養管理の実態調査
データベースから取得可能な処方や治療の情報を用いて、下記の項目のような栄養管理の実態を調査する。
・栄養投与経路の推移
・静脈栄養処方量の推移
・処方輸液製剤の種類
・静脈栄養処方量の分布
(2) 急性期の補足的静脈栄養と予後の関連/栄養処方量と予後の関連
多変量回帰モデルにより交絡因子となる患者背景で調整したオッズ比と95％信頼区間を推定することで、補足的静脈栄養や栄養処方量と臨床転帰の関連を検討する。
(3) 回復期の栄養管理・リハビリテーションの実態調査
データベースから取得可能な処方や治療、リハビリテーションの情報を用いて、下記の項目のような実態を調査する。
・栄養投与経路の推移
・栄養介入とリハビリテーションの実施状況
・Functional Independence Measure（FIM）の推移
・退棟時FIM関連因子の検討

A cohort study
Data source
A medical database by Medical Data Vision Co. Ltd. The database is based on the DPC data/medical claims database and covers approximately 32% of all DPC hospitals in Japan. All data is anonymized for personal information protection.
(1) Survey of acute-phase nutritional management
Nutritional management during the acute phase will be assessed using prescription and treatment data available in the database. The following items will be analyzed:
･Trends in routes of nutritional administration
･Trends in prescribed doses of parenteral nutrition
･Types of prescribed infusion preparations
･Distribution of prescribed doses of parenteral nutrition
(2) Association between supplemental parenteral nutrition / nutritional prescription doses and clinical outcomes
The associations of supplemental parenteral nutrition and nutritional prescription doses with clinical outcomes will be evaluated using multivariable logistic regression. Adjusted odds ratios and 95% confidence intervals will be estimated after adjustment for patient characteristics as confounders.
(3) Survey of nutritional management and rehabilitation in the convalescent ward
Nutritional management and rehabilitation in the convalescent ward will be assessed using prescription, treatment, and rehabilitation data available in the database. The following items will be analyzed:
･Trends in routes of nutritional administration
･Implementation status of nutritional interventions and rehabilitation
･Trends in Functional Independence Measure (FIM) scores
･Factors associated with FIM at discharge
