本研究の目的は、日本における違法薬物を対象とした下水疫学調査（Wastewater-Based Epidemiology: WBE）に対する一般住民の社会的受容性を明らかにすることである。関東地域在住の18歳以上一般住民を対象に横断的オンラインアンケート調査を実施し、違法薬物の使用状況把握を目的とした下水調査に対する受容性、主な懸念、調査結果の利用範囲に対する考え、違法薬物使用者に対する規範意識、下水調査に関する事前認知、違法薬物問題への関心、行政・研究機関への信頼等を探索的に検討する。本研究により、日本で違法薬物を対象としたWBEを導入・社会実装する際に必要となる説明方法、制度設計、データ利用方針、倫理的配慮等を検討するための基礎資料を得ることを目指す。

The objective of this study is to examine social acceptability of wastewater-based epidemiology (WBE) for illicit drug monitoring among the general adult population in Japan. A cross-sectional online questionnaire survey will be conducted among adults aged 18 years or older living in the Kanto region. The study will explore acceptability of WBE for monitoring illicit drug use, major concerns, views on the use of survey results, normative attitudes toward people who use illicit drugs, prior awareness of wastewater monitoring, interest in drug-related issues, and trust in government and research institutions. The findings are expected to provide basic evidence for considering explanations, institutional design, data-use policies, and ethical safeguards needed for the future social implementation of WBE for illicit drug monitoring in Japan.

主要アウトカムは、違法薬物の使用状況把握を目的とした下水調査に対する受容性である。受容性は、単回のオンラインアンケート調査時点において、以下の3項目により5件法で評価する。

1. 総合的受容性：違法薬物の使用状況を地域全体で把握するための下水調査という考え方そのものへの賛否
2. 自地域での受容性：自分が居住する地域で実施される場合の受け入れ可能性
3. 公衆衛生目的での受容性：健康課題の把握や予防教育、依存症患者への支援等の公衆衛生対策につなげる目的で実施される場合の受け入れ可能性

The primary outcome is acceptability of wastewater monitoring for assessing illicit drug use. Acceptability will be measured at the time of a single online questionnaire survey using three five-point Likert-type items
1.Overall acceptability: agreement with the concept of wastewater monitoring to assess trends in illicit drug use at the community level.
2.Local acceptability: acceptability of implementing such monitoring in the respondent's own residential area.
3.Public-health-oriented acceptability: acceptability of implementing such monitoring when it is intended to inform public health measures, such as prevention education and support for people with substance use problems.

Key secondary outcomes are factors and attitudes potentially associated with acceptability of wastewater monitoring for illicit drug use.
These include prior awareness of wastewater monitoring, views on how the results should be used, major concerns, concerns about the image of the respondent's residential area, normative attitudes toward people who use illicit drugs, interest in illicit drug use and drug dependence issues, trust in government and research institutions, educational attainment, occupation, age, sex, and residential area.

本研究で得られた個人を直接特定できないアンケート回答データは、将来的に、違法薬物を対象とした下水疫学調査の社会的受容性、倫理的課題、制度設計、公衆衛生政策等に関連する研究に二次利用される可能性がある。ただし、二次利用または他機関への提供を行う場合は、必要に応じて倫理委員会で改めて審査・承認を受け、適切な手続きに基づいて実施する。一般公開用の個別参加者データセットとして共有する予定はない。

本研究は、関東地域在住の18歳以上一般住民を対象とする前向き横断的オンラインアンケート調査である。オンライン調査会社の登録モニターから、地域、性別、年代による割付に基づき、1,200から1,500名程度を募集する。地域区分は、東京都、神奈川・埼玉・千葉、茨城・栃木・群馬の3区分とし、年代は18歳から29歳、30歳から39歳、40歳から49歳、50歳から64歳、65歳以上の5区分とする。結果変数は、違法薬物の使用状況把握を目的とした下水疫学調査への受容性であり、総合的受容性、自地域での受容性、公衆衛生目的での受容性を5件法で評価する。関連要因として、下水調査の事前認知、調査結果の利用範囲に対する考え、主な懸念、地域イメージへの懸念、違法薬物使用者に対する規範意識、違法薬物問題への関心、行政・研究機関への信頼、年齢、性別、居住地域、最終学歴、職業等を検討する。解析では、各設問の回答分布を記述し、受容性と関連要因との関連について、クロス集計、カイ二乗検定、必要に応じてロジスティック回帰分析等を用いて探索的に検討する。本研究は単回調査であり、介入、検査、医薬品・医療機器の使用、個人への追跡は行わない。オンラインアンケート調査の実施、対象者募集、回答収集およびデータ提供はオンライン調査会社に委託する。研究者には、氏名、住所、電話番号、メールアドレス等の個人を直接特定できる情報は提供されない。

This is a prospective cross-sectional online questionnaire survey of general adult residents aged 18 years or older living in the Kanto region of Japan. Approximately 1,200 to 1,500 participants will be recruited from registered monitors of an online survey company using quota sampling by region, sex, and age group. The regional categories are Tokyo; Kanagawa, Saitama, and Chiba; and Ibaraki, Tochigi, and Gunma. Age groups are 18-29, 30-39, 40-49, 50-64, and 65 years or older. The outcome is acceptability of wastewater-based epidemiology for monitoring illicit drug use, assessed using five-point Likert-type items on overall acceptability, acceptability in the respondent's own residential area, and acceptability for public health purposes. Potentially related factors include prior awareness of wastewater monitoring, views on the use of survey results, major concerns, concerns about the image of the respondent's residential area, normative attitudes toward people who use illicit drugs, interest in illicit drug use and drug dependence issues, trust in government and research institutions, age, sex, residential area, educational attainment, and occupation. The analysis will describe response distributions and exploratorily examine associations between acceptability and related factors using cross-tabulations, chi-square tests, and logistic regression analyses as appropriate. This is a single-time-point questionnaire survey and involves no intervention, clinical examination, use of medical products or devices, or individual follow-up. The online survey company will conduct participant recruitment, questionnaire administration, response collection, and data provision. The researchers will not receive directly identifiable personal information, such as names, addresses, telephone numbers, or email addresses.
